Position Summary
BrightCare Home Care is seeking a highly motivated, self-driven, and compassionate Part-Time Case Manager / Community Liaison to join our growing team. This position is responsible for developing community partnerships, managing referral leads, and guiding prospective clients through the intake process.
The ideal candidate is energetic, goal-oriented, and enjoys connecting with people. They will be responsible for building referral relationships throughout the community, managing leads from A Place for Mom and other referral sources, and converting those leads into clients through consistent follow-up and outstanding customer service.
This is an excellent opportunity for someone who has experience in the home care or healthcare industry and is passionate about helping seniors and their families access quality care.
Essential Responsibilities
- Develop and maintain strong referral relationships with hospitals, rehabilitation facilities, physician offices, senior living communities, social workers, discharge planners, case managers, and other healthcare professionals.
- Represent BrightCare Home Care at community networking events, health fairs, and outreach activities to promote the agency's services.
- Receive and promptly respond to referral leads from A Place for Mom and other referral sources.
- Contact prospective clients and families to assess their needs and educate them on BrightCare Home Care's services.
- Consistently follow up with referral leads to build relationships and convert inquiries into active clients.
- Coordinate the client intake process and ensure a positive experience from the initial inquiry through the start of services.
- Maintain accurate documentation of all referrals, client communications, outreach activities, and follow-up efforts.
- Work collaboratively with agency leadership to identify opportunities for business development and community engagement.
- Meet established referral, outreach, and client conversion goals while delivering exceptional customer service.
Minimum Qualifications
- Minimum of two (2) years of experience in the home care, senior care, or healthcare industry is required.
- Previous experience in case management, client services, admissions, community outreach, healthcare marketing, or business development is strongly preferred.
- Demonstrated experience building professional relationships and generating referrals.
- Excellent communication, customer service, and interpersonal skills.
- Strong organizational and time management abilities with the ability to work independently.
- Comfortable making outbound calls and conducting ongoing follow-up with prospective clients and referral partners.
- Proficiency with Microsoft Office, email, and electronic client management systems.
- Valid driver's license, reliable transportation, and willingness to travel locally for community outreach activities.

Schedule & Compensation
- Part-Time: 16 hours per week
- Compensation: $20.00 per hour
- Flexible schedule based on business and community outreach needs.
