Full Job Description

Duties

Help people with disabilities develop functional skills, such as personal hygiene, bathing, meal preparation, dressing, and toileting. The approach should be instructional and tailored to each person's needs and goals.

Assist individuals in developing appropriate interpersonal interactions and social situations, encouraging personal growth and normalized socialization.

Create activities, situational experiences, or leisure time for people supported using necessary equipment, vehicles, events, or sites following a daily routine, written schedule, or ISP goals, according to the person's interests.

Accompany individuals to medical appointments, community outings, or events away from home using necessary personal vehicles (with mileage reimbursement), company vehicles, or public transportation following each person's needs, goals, and planned events or in emergencies following instructions from Nurse or Supervisory personnel.

Discuss the response, reaction, and progress of the person supported by the House Manager, Supported Living Manager, and other supervisory staff in line with agency requirements and state or federal guidelines.

Collaborate with the support team, including relatives, service coordinators, day program staff, treatment professionals, and other support team members, to ensure the health, safety, and welfare of the person supported.

Provide Behavioral and Medical Interventions as described in behavioral support plans, nursing plans, nutrition plans, and other treatment-related documentation.

Encourage using learned skills and abilities in the natural daily routine to maximize normalized development and independent functioning.

Perform health-related tasks, such as giving prescribed medications (as a certified TME), taking blood pressure/temperature, checking skin condition, colostomy care assistance, positioning, using prescribed procedures, or following QIDP/Nurse instructions to maintain each person's health, safety, and welfare.

Participate in all required training, including Phase I, II, III, and IV. Maintain state and federal certifications, including First Aid, CPR, TME, and BPS/ Mandt/ CPI.

Ensure that annual requirements for PPD are completed.

Maintain knowledge of treatment plans for the person supported. Exchange and receive information about the person in verbal or written format as required to maintain records, treatment plans, or implement services, including developing daily shift notes and logging behavioral and goal data.

Participate in various regularly scheduled duties, including household chores, emergency drills, meal preparation, grocery shopping, etc., following assigned schedules and regulations to ensure the health, safety, and welfare of the people supported.

Substitute for absent or unavailable co-workers in performing assigned duties with direction from the supervisor to ensure the day-to-day functioning of the program.

Perform miscellaneous duties as assigned by the supervisor to ensure the day-to-day functioning of the program.
