Primary Function:

PMHW is seeking a Case Management Lead to support the development, implementation, and growth of a newly developing QMHA Case Management Services program within our outpatient behavioral health organization.

This role is ideal for an individual who is excited about helping build infrastructure, shape workflows, and contribute to the long-term development of care coordination services across the organization. PMHW currently has foundational components of the program identified, including referral concepts, organizational structure, and service goals, and is seeking a leader who can help operationalize, strengthen, and expand the program over time.

The Case Management Lead will provide direct QMHA Case Management services while also taking ownership of workflow development, referral coordination, interdisciplinary collaboration, and program growth initiatives. This role will work closely with leadership to help establish sustainable systems that support patients, providers, and organizational needs.

As the program grows, this position may also support onboarding, mentorship, and supervision-related coordination for future staff, trainees, and/or students participating in QMHA-aligned learning opportunities.

This position requires a strong balance of: direct patient support, operational thinking, initiative, leadership, collaboration, and adaptability within a growing program environment.

Essential Functions:

Program Development & Leadership

Support ongoing development and implementation of PMHW’s Case Management Services program
Help refine workflows, referral processes, documentation standards, and operational systems
Assist in identifying program needs, workflow gaps, and opportunities for improvement
Take initiative in organizing and strengthening day-to-day Case Management operations
Collaborate with leadership regarding program growth, staffing needs, and service development
Support long-term scalability and sustainability of Case Management services across departments
Help establish structure for future onboarding and training processes

Direct Case Management Services

Provide direct patient-facing Case Management services
Support patients with care coordination, resource navigation, and barrier reduction
Assist patients in accessing community resources and behavioral health supports
Coordinate with internal and external providers/systems as appropriate
Support continuity of care across outpatient, IOP, psychiatric, and community-based services
Provide follow-up support related to patient engagement and treatment participation
Complete documentation in alignment with OAR, Medicaid, COA, and PMHW standards

Referral Coordination & Workflow Oversight

Assist in managing incoming Case Management referrals through the CM URF process
Help triage referrals based on level of need and service type
Coordinate services across departments and providers
Monitor referral trends, service utilization, and workflow efficiency
Assist with development of tracking and productivity processes

Interdisciplinary Collaboration

Collaborate closely with: Outpatient Psychotherapy, Training Department, Intensive Outpatient Program (IOP), Psychiatric Services, PCC/Administrative teams, Leadership and compliance staff
Support communication and coordinated care efforts across the organization

Future Workforce Development Support

As the program develops, this role may assist with: Supporting future QMHA staff onboarding, Mentoring newer staff, Assisting with student and trainee workflow integration, Helping develop learning opportunities related to care coordination and behavioral health support services
Supporting a collaborative and growth-oriented team culture

Minimum Qualifications:

QMHA I or II or Board Certified under Oregon standards required (LCSW, LMSW, LMFT, LPC, Psychologist or associates)
2+ years experience providing behavioral health case management, care coordination, or community support services required
Knowledge of Oregon behavioral health systems and community resources
Experience working with Medicaid populations
Experience with OAR, Medicaid, and/or COA-related standards
Experience in outpatient behavioral health settings
Prior leadership, program development, or supervisory experience
Experience supporting workflow development or operational improvement initiatives
Bilingual or multilingual skills preferred
Experience working with trainees, interns, or students preferred
Strong understanding of trauma-informed and culturally responsive care practices
Strong organizational skills and attention to detail
Ability to handle sensitive and confidential information with discretion
Familiarity with diversity, equity, access, and inclusion (DEAI) principles, and anti-racism and anti-oppression practices
Experience with and ability to use systems thinking
Ability to work with Mac OS & Google Workspace
