Overview

In this role, the Case Manager provides field based services (community, home, or office based) to children, families, and caregivers served through Olive Crest’s Adoption Promotion Support Services (APSS) program. The Case Manager supports each referred APSS placement with the goal of achieving long-term stability and permanence by assessing needs, coordinating and linking services, collaborating with community providers, participating in Child and Family Team meetings, and supporting discharge planning.

Job Duties and Responsibilities:

Manage and supervise a caseload of 23 families/clients, and facilitate monthly, face-to-face home visits scheduling to best meet the needs of the client and family.
Conduct intake assessments and collaborate with families to develop and implement Individualized Service Plans (ISPs).
Provide intensive, home-based mental health services as determined by the treatment team.
Respond to and support families during crisis situations, including after-hours response when required.
Cultivate contacts and relationships with outside agencies and individuals that have both direct and indirect bearing on the case (e.g., County Social Workers, School Personnel, Regional Center Personnel, etc.)
Coordinate and monitor referrals and linkages to community resources to promote family stability and well-being.
Participate in client-focused meetings (e.g., IEPs, CFTs, Wraparound meetings, psychiatry appointments) as needed.
Maintain timely, accurate clinical documentation, including progress notes, assessments, incident reports, and discharge summaries in compliance with mental health and contract requirements.
Collaborate with county agencies, schools, and community providers to support coordinated care.
Attend required treatment team meetings, supervision, training, and support groups.

Required Qualifications:

Bi-lingual (English and Spanish)
MUST hold a bachelor’s degree from an accredited college or university in one of the following: social work; marriage, family and child counseling; child psychology; child development; counseling psychology; social psychology; clinical psychology; educational psychology; or education with emphasis on counseling.
Experience working within child welfare and family systems, and/or trauma-informed work.
Adoption-related experience is preferred.
Must obtain and maintain record clearance free from criminal convictions.
Negative Mantoux PPD Test/chest x-ray not more than one year prior to commencing work and every three (3) years thereafter confirming employee is free of tuberculosis.
Must have CPR and First Aid certification upon hire and maintained throughout employment.
Must have and maintain a valid California Driver License and clean driving record in accordance with Olive Crest’s auto insurance policy.
Must maintain vehicle insurance throughout course of employment.
Comfortable spending significant time driving as part of regular duties.
Must have a philosophy of support for abused children in line with values, mission and treatment philosophy of Olive Crest.

Preferred Qualifications

Able to work both independently with minimal supervision and collaboratively as part of a team.
Computer skills necessary (proficiency in Word, Excel, Outlook, Adobe).
Demonstrates flexibility, dependability, professionalism, and strong communication skills (verbal and written).
Exercise good judgment, maturity, and problem-solving ability in situations where limited guidelines exist.
Capable of interpreting and following written, verbal, diagrammatic, or schedule-based instructions.
