Full Job Description

We're looking for an experienced Wellness Nurse (LPN or RN) to join our caring clinical team part-time, with the potential to increase hours down the line. Competitive Pay - $30- 34 per
- The starting rate is based on experience and This position participates in our compensation program, which provides opportunities for future pay growth based on performance and career development.

What You'll Do

Complete nursing assessments and monitor residents' health and overall well-being
Observe, document, and communicate changes in resident condition
Safely administer and document medications while following nursing best practices
Provide guidance, education, and clinical support to caregiving staff
Assist with resident care planning and collaborate with the interdisciplinary care team
Partner with physicians, therapists, pharmacies, hospice providers, and other healthcare professionals to coordinate resident care
Respond professionally to resident and family questions and concerns
Maintain accurate clinical documentation and medication records
Ensure compliance with state regulations, company policies, and nursing standards
Protect resident confidentiality while promoting a safe, compassionate environment

What We're Looking For

Current LPN or RN license in the State of Utah
Knowledge of quality nursing practices and assisted living regulations
Previous experience in assisted living, senior living, long-term care, rehabilitation, memory care, or skilled nursing preferred
Strong clinical assessment, critical thinking, and problem-solving skills
Ability to lead, coach, and support caregiving staff while fostering teamwork
Excellent communication, organization, and documentation skills
Compassionate, dependable, and committed to providing exceptional care for seniors
