Position Overview: As a Skilled Nurse Case Manager, you will be a key player in our patient care team, managing and coordinating comprehensive care plans for patients in their homes. This role offers flexibility, typically working Monday through Friday, 8:00 am to 4:30 pm, with minimal to no nights, weekends, or holidays.

Key Responsibilities:

Develop, implement, and manage individualized care plans.
Conduct patient assessments and monitor progress.
Coordinate care with interdisciplinary teams including physicians and therapists.
Educate patients and their caregivers and support team about health conditions and care plans.
Ensure compliance with all regulatory standards and company policies.
Maintain accurate and thorough documentation.

Qualifications:

Valid RN license in the state of Iowa.
Reliable transportation and a valid driver’s license.
Strong communication and organizational skills.
Ability to work both independently and collaboratively.
A passion for providing home care and supporting mental/behavioral health needs.
