Optimum Choice Services, Inc. is a premiere provider that has been helping people with traumatic brain injuries, chronic medical and developmental diagnoses since 2008. We are currently looking to fill Independent Living Skills Trainer (ILST) positions in NYC Metro area, Lower Hudson Valley and Long Island Regions for part time/per diem shifts. Flexible shifts are available for weekends and evenings, as a secondary form of employment.

Overview: Independent Living Skills Trainer (ILST) develop services individually designed to improve the ability of our participants to live as independently as possible within the community. As an ILST you will be responsible for the training, supervision of, or assistance to, an individual with self-care including: medication management training, task completion, communication & interpersonal skills, socialization, sensory & motor skills, mobility, community transportation skills, problem solving, money management, ability to maintain a household. This position is not direct services/home care. No toileting, transferring, medical care provided. This is a person-centered driven service that trains and assists clients with their personal goals for increased independence in a community setting.

ILSTs will be required to provide this service in the client's home and/or community. Additional job responsibilities are carried out remotely including but not limited to note completion within 72 hours of the service session; supervision sessions; and mandatory meetings or trainings.

QUALIFICATIONS: Persons employed as an Independent Living Skills and Training provider must be a/an:

Be an individual with an Associate’s degree and five (5) years of experience completing assessments of ADLs and IADLs, developing a comprehensive service plan, teaching individuals with Traumatic Brain Injury to be more functionally independent. Or;
Be an individual with a Bachelor’s degree and three (3) years of experience completing assessments of ADLs and IADLs, developing comprehensive service plans and teaching individuals with Traumatic Brain Injury or chronic medical diagnosis to be more functionally independent; Or;

AND

Must have, at a minimum, one (1) year of experience completing assessments of ADLs/IADLs, developing a comprehensive service plan, teaching individuals with Traumatic Brain Injury or developmental disabilities to be more functionally independent;

*Any currently employed ILST Provider who met waiver application qualifications upon hire will be issued a certificate allowing them to continue providing the same service(s) at both their current location of employment and/or any change they have in future employment locations/agencies. This certificate has no expiration date and only applies to the waiver service they were qualified for upon hire. Certificates may be terminated at the discretion of NYSDOH.

DUTIES AND RESPONSIBILITIES:

Provide training to participants in community settings which may include that of medical appointments, food stores and/or other essential events. Training must be directly related to the participant’s goal plan with the outcome being that of skill development.
Develop and implement Detailed Goal Plans derived from the functional assessment by meeting with each participant as indicated in their service plan and monitoring progress on goals.
Complete progress notes in Medisked within 72 hours of the session with a participant. This includes additional required documentation which may be needed by your supervisor. Progress notes document date, time and duration of the session and summarize essential information that promotes consistency with care.
Complete progress notes as needed for contact with Service Coordinator and other service providers.
Complete Individual Service Reports (ISR) as required to document goals, progress, and recommendations.
Attend and actively participate in team meetings for assigned participants. Team meetings are typically scheduled by the Service Coordinator and are designed to promote treatment coordination by review of the current service plan, stated goals and outcomes. The ILST is expected to notify their supervisor in the event they are unable to attend a scheduled meeting so that coverage by the supervisor can be arranged.
Provide training as needed to all individuals involved in supporting the participant to continue independence on achieved goals (i.e., caregivers, in-home workers, waiver providers, etc.).
Interface with the Service Coordinator and all other service providers to maintain communication regarding the progress of each participant served.
Meet with supervisor at least bi-weekly for regular supervision and feedback.
Attend and complete training in accordance with New York State regulations and Agency policy.
Perform additional responsibilities as requested and/or necessary, including staffing alternate locations when assigned.
Collaborate with all disciplines to plan and evaluate team goals for each participant.
Adhere to all policies and procedures when providing services to participants.
Complete all necessary trainings to maintain employment.
Perform other duties as assigned.
