BMC2 is a collaborative consortium dedicated to improving the quality of care and outcomes in patients who undergo cardiovascular procedures across the State of Michigan. BMC2 is developing a new heart failure Quality Improvement Program, which will take a population health-based approach to measuring outcomes and improving quality of care for patients with heart failure. The new program will bring together diverse stakeholders including patients, specialists, primary care physicians, and pharmacists. Support for BMC2 is provided by Blue Cross Blue Shield of Michigan and Blue Care Network as part of the BCBSM Value Partnerships Program.
The Patient Engagement Coordinator will manage patient engagement activities for BMC2, including the new BMC2 Heart Failure Quality Improvement Program. This role will support the integration of patient and caregiver perspectives into program development, quality improvement priorities, patient-reported measurement, social impact initiatives, educational materials, and dissemination efforts. Responsibilities include coordinating engagement workstreams, supporting patient and caregiver advisors, managing stakeholder relationships, facilitating advisory council activities, developing engagement materials including a patient stories library, supporting participating sites, tracking deliverables, and evaluating the impact of patient engagement efforts.
This role will report to the BMC2 Community Engagement Manager and will work collaboratively with core program leadership, external partners, and current members of the BMC2 team.
Project Management and Program Implementation
- Execute the day-to-day patient engagement workstream for BMC2 Heart Failure
- Support implementation and continuous improvement of patient engagement processes, tools, workflows, and best practices across BMC2.
- Prepare project plans, status updates, reports, presentations, meeting materials, and leadership updates related to patient engagement activities.
- Assist with operational logistics related to scheduling, stipends, reimbursements, vendor coordination, budget tracking, and other project needs as assigned.
Patient and Caregiver Engagement and Advisory Council Support
- Recruit, onboard, orient, and support patient and caregiver advisors across BMC2 programs, ensuring meaningful participation in registry and program development, quality improvement, and consortium activities.
- Coordinate patient advisory council activities, including planning, facilitation, agendas, materials, documentation, action items, and follow-up for advisory council meetings, listening sessions, interviews, focus groups, and workgroups.
- Track patient and caregiver recommendations and maintain feedback loops so advisors understand how their input influenced decisions, materials, measures, and quality improvement priorities.
- Support preparation of patient and caregiver advisors for participation in meetings, panels, webinars, learning collaboratives, presentations, and other BMC2 engagement activities.
Stakeholder Coordination, Site Support, and Consortium Collaboration
- Serve as a liaison and relationship manager among patients, caregivers, BMC2 leadership, clinicians, quality improvement staff, data teams, participating sites, and community partners.
- Support BMC2-participating sites by incorporating patient engagement into local quality improvement activities, including learning collaboratives, webinars, site meetings, consortium events, and other shared learning opportunities.
- Coordinate communication and collaboration across multidisciplinary stakeholders to support alignment of patient engagement goals across BMC2.
- Escalate stakeholder concerns, implementation barriers, or site needs to appropriate leadership.
Integration of Patient Perspectives into Program Design, Measurement, and Quality Improvement
- Coordinate patient and caregiver input into BMC2 Heart Failure Quality Improvement Program development, including registry measures, data elements, workflows, patient-reported outcome tools, and patient-facing processes.
- Support patient engagement in quality improvement initiatives across BMC2, with initial emphasis on heart failure care transitions, readmissions, medication optimization, follow-up care, and patient education, and additional support for PCI and vascular surgery activities as assigned.
- Work with data, registry, and quality improvement teams to interpret findings through a patient-centered lens and translate patient experience themes into actionable improvement opportunities.
Evaluation, Compliance, and Ethical Engagement
- Identify and address barriers to participation, including barriers related to language, disability, geography, transportation, technology access, health literacy, insurance status, and social factors.
- Develop and monitor metrics to evaluate the reach, effectiveness, and impact of patient engagement efforts.
- Synthesize qualitative feedback from patients and caregivers and prepare evaluation summaries for leadership, consortium stakeholders, grants, annual reporting, or other reporting needs.
- Ensure patient engagement activities comply with applicable BMC2, HIPAA, privacy, confidentiality, and data governance requirements.
- Maintain appropriate role boundaries and refer clinical concerns to licensed clinical professionals.
- Bachelors degree or equivalent combination of education and experience.
- 2-4 years relevant experience in quality improvement, patient experience, patient engagement, community engagement, or a related field.
- Demonstrated ability to manage project timelines, deliverables, meetings, documentation, and stakeholder communication.
- Excellent written, verbal, and interpersonal communication skills.
- Ability to translate complex information into clear, patient-centered, plain-language materials.
- Strong organizational skills and attention to detail, with the ability to manage multiple priorities.
- Experience building trust and working collaboratively with diverse stakeholders, including patients.
- Demonstrated commitment to patient-centered care, positive social impact, and accessibility
- Ability to synthesize information, summarize feedback, track action items, and prepare reports or presentations.
- Familiarity with privacy and confidentiality expectations in health care or related settings.
- Proficiency with standard office, virtual meeting, and project coordination tools.
