About Sentry Living Solutions

Sentry Living Solutions provides person centered services to individuals with intellectual and developmental disabilities and their families throughout Northern California. As a California Regional Center vendor, we work collaboratively with individuals, families, Regional Centers, generic agencies, and community providers to promote independence, inclusion, stability, and meaningful life outcomes.

Position Summary

The Bilingual Coordinated Family Support Specialist provides ongoing, field based support to adults with developmental disabilities who live in their family homes.

This is not primarily an office or administrative position. Most working hours will involve traveling to meet individuals and families in their homes and communities, helping them implement their CFS Plan of Action, navigate generic and community resources, and overcome barriers to obtaining needed services.

The Specialist will manage an assigned caseload under the supervision of the CFS Program Coordinator. The Program Coordinator will complete initial assessments, develop and oversee service plans, assign cases, provide supervision, and address complex matters. The Specialist will provide consistent follow-up and ongoing implementation of authorized CFS services.

Services must be individualized, person centered, culturally responsive, and respectful of each individual’s and family’s language, preferences, and circumstances.

Essential Responsibilities Ongoing Individual and Family Support

Manage an assigned caseload of individuals receiving CFS services.
Conduct regular visits in individuals’ homes and communities.
Build respectful and professional relationships with individuals and their families.
Implement assigned activities and goals identified in each individual’s CFS Plan of Action.
Provide services in Spanish or English according to the family’s preferred language.
Maintain consistent communication and follow-up with individuals and families.
Identify changes in needs, barriers, risks, or family circumstances and report them promptly to the Program Coordinator.
Refer complex concerns, service-plan changes, and urgent matters to the Program Coordinator.

Generic and Community Resource Navigation

Help individuals and families understand, locate, and access available generic and community resources.
Assist families in navigating programs such as Medi-Cal, SSI, IHSS, CalFresh, transportation, healthcare, behavioral health, housing resources, educational or vocational services, and local community programs, as applicable.
Help families gather information, organize required documents, complete follow-up tasks, and prepare for appointments.
Identify barriers preventing access to services and work with the family and Program Coordinator to develop practical solutions.
Provide education and support without replacing the responsibilities of Regional Center service coordinators, generic agencies, or other authorized providers.
Help individuals access meaningful community activities and opportunities.

Service Coordination and Follow-Up

Assist with scheduling and coordinating medical appointments, service appointments, provider meetings, and other supports identified in the CFS Plan of Action.
Help identify transportation options and address transportation barriers.
Communicate professionally with Regional Center service coordinators, healthcare professionals, generic agencies, and community providers as authorized.
Follow up on referrals, applications, appointments, and outstanding service needs.
Help identify backup resources and support options when an existing service plan is not working.
Support future planning, particularly for individuals living with aging caregivers.
Reinforce skills and strategies that promote the individual’s independence and continued success in the family home.

Documentation and Reporting

Complete accurate and timely case notes for every service provided.
Document activities, barriers addressed, progress toward goals, and outcomes achieved.
Maintain complete and confidential electronic client records.
Track authorized hours and manage services within each client’s approved authorization.
Provide timely case updates to the Program Coordinator.
Contribute information needed for quarterly CFS progress reports.
Participate in case reviews, supervision, staff meetings, and required training.
Immediately report health, safety, abuse, neglect, or other urgent concerns according to agency and mandated-reporting procedures.

Required Qualifications

Candidates must meet the applicable California Department of Developmental Services minimum qualifications for CFS staff through one of the following:

An associate-level degree in human services or a related field; or
At least three years of experience in the developmental-disability service-delivery system.

Candidates must also have:

Fluency in Spanish and English, including the ability to communicate professionally with Spanish speaking individuals and families.
Knowledge of the California Regional Center system.
Experience working with individuals with intellectual or developmental disabilities and their families.
Ability and willingness to travel regularly to assigned client homes and community locations.
Reliable transportation and the ability to meet the agency’s driving and insurance requirements.
Ability to work independently in the community while maintaining regular communication with the Program Coordinator.
Ability to maintain confidentiality and comply with HIPAA, agency policies, documentation standards, and program requirements.
Strong written communication, organization, time management, and follow-through.
Ability to use email, electronic records, Google Workspace, and Microsoft Office.

Preferred Qualifications

Degree or coursework in social work, psychology, human services, sociology, special education, or a related field.
Experience providing CFS, case management, resource navigation, family support, independent living services, or similar community based services.
Familiarity with CFS Plans of Action, IPPs, Regional Center authorizations, and quarterly reporting.
Knowledge of generic resources such as SSI, IHSS, Medi-Cal, CalFresh, transportation programs, behavioral health services, housing resources, and local Area Agencies on Aging.
Experience working with culturally and linguistically diverse families.
Familiarity with mandated reporting and person-centered service practices.

Schedule and Travel

Part-time, up to 30 hours per week.
Schedule will be based on assigned family needs, appointments, authorized service hours, and travel requirements.
Some late-afternoon or early-evening availability may be required to accommodate family schedules.
Regular travel to client homes and community locations is an essential function of the position.
Office attendance may be required for supervision, meetings, documentation, and training.
Travel reimbursement will be provided in accordance with company policy and applicable law.

Physical and Work Requirements

Ability to travel between client homes and community locations.
Frequent speaking, listening, walking, standing, sitting, driving, and computer use.
Ability to work in varied home and community environments.
Ability to occasionally lift or carry materials weighing up to 25 pounds, with or without reasonable accommodation.

Training and Supervision

Program-specific training and ongoing supervision will be provided. Required training may include CFS requirements, person-centered practices, documentation, confidentiality, mandated reporting, behavioral health resources, generic service systems, local Area Agencies on Aging, and agency policies.
