Full Job Description

LOCATION: Remote – must live or near the following North Carolina counties: Alexander, Anson, Cabarrus, Catawba, Cleveland, Gaston, Iredell, Lincoln, Mecklenburg, Rowan, Stanly, or Union. This person is required to maintain residency in North Carolina or within 40 miles of the NC border. This work may require travel across these counties.

GENERAL STATEMENT OF JOB

The Rural Health Transformation Program (RHTP) is statewide program administered by the NC Department of Health and Human Services (DHHS). The purpose of the RHTP is to create sustainable improved access and outcomes to NC citizens living in rural communities. This program is funded by the US Centers for Medicare & Medicaid Services (CMS). The ROOTS Hub serves as the regional coordinating entity responsible for implementing RHTP priorities throughout Region 3.

The ROOTS Hub Initiatives Manager reports to the ROOTS Hub Director and provides operational leadership, coordination, and oversight across all assigned RHTP initiatives. This position supervises the Initiative 1 Co-Lead, Initiative 2 Lead, and Initiative 5 Lead, ensuring alignment of implementation activities, consistency in project management, and successful achievement of Hub goals and deliverables.

The Initiatives Manager serves as the primary operational liaison between the ROOTS Hub and the Region 3 Academic Anchor, Regional Anchors, implementation partners, and other key stakeholders. This position is responsible for coordinating cross-initiative planning, monitoring implementation progress, facilitating communication across partner organizations, identifying operational risks, and supporting execution of the Region 3 Hub Action Plan.

Working closely with the ROOTS Hub Director, this position translates strategic priorities into coordinated implementation activities while promoting collaboration, accountability, and continuous improvement across the Hub.

ESSENTIAL JOB FUNCTIONS

Cross-Initiative Leadership and Organizational Oversight:

Provide direct supervision, coaching, and performance management for Initiative Leads.
Coordinate implementation across Initiatives 1, 2, and 5 to ensure alignment with the Region 3 Hub Action Plan.
Monitor initiative work plans, milestones, deliverables, and implementation schedules.
Identify interdependencies across initiatives and proactively address operational barriers.
Ensure consistency in implementation approaches, documentation, and project management practices.

Anchor Coordination and Strategic Partnerships:

Serve as the primary operational liaison to the Region 3 Academic Anchor and Regional Anchors.
Coordinate regular implementation meetings with anchor organizations and implementation partners.
Foster collaborative relationships among healthcare providers, community organizations, local health departments, academic institutions, and state partners.
Facilitate communication between initiative teams and external partners to ensure alignment of priorities and activities.
Support development of collaborative strategies that advance regional transformation goals.

Program Management, Reporting & Performance Monitoring:

Monitor initiative performance metrics and implementation progress across all assigned initiatives.
Review status reports, dashboards, and performance data to identify risks and opportunities.
Support preparation of reports, presentations, and implementation updates for NCDHHS, Hub leadership, and governance bodies.
Ensure timely completion of deliverables and compliance with RHTP requirements.
Coordinate corrective action planning when implementation challenges arise.

Governance and Continuous Improvement:

Support Hub governance activities, including Regional Decision Council meetings, implementation workgroups, and advisory committees.
Coordinate cross-functional planning sessions and implementation reviews.
Promote continuous quality improvement and knowledge sharing across initiative teams.
Support development and refinement of operational processes and implementation tools.

Other Duties:

Provides direct supervision to assigned Initiative Leads, including:

Initiative 1 Co-Leads

Initiative 2 Lead

Initiative 5 Lead

Provides leadership through coaching, performance management, workload prioritization, professional development, and operational oversight.

KNOWLEDGE, SKILLS, & ABILITIES

Strong leadership and supervisory skills.
Advanced project and program management experience.
Knowledge of healthcare delivery systems, behavioral health, public health, and social determinants of health.
Understanding of rural healthcare transformation strategies.
Experience coordinating large, multi-organizational initiatives.
Ability to build collaborative relationships across diverse stakeholder groups.
Strong facilitation and meeting management skills.
Excellent written and verbal communication skills.
Ability to analyze performance data and translate findings into operational improvements.
Knowledge of grant-funded initiatives and public-sector accountability.
Ability to manage competing priorities while maintaining attention to detail.
Proficiency with Microsoft Office Suite and project management software.

EDUCATION & EXPERIENCE REQUIREMENTS

Bachelor’s degree in public health, healthcare administration, social work, business administration, public administration, or related field required. Master’s degree preferred.

Minimum of five (5) years of progressively responsible experience leading complex healthcare, public health, or population health initiatives.
Minimum of three (3) years of project or program management experience involving multiple organizations or stakeholder groups.
Minimum of two (2) years of supervisory or team leadership experience.

Experience coordinating regional partnerships and collaborative initiatives.

Preferred work experience:

Experience with Medicaid, managed care, or state-funded healthcare initiatives.
Experience working with rural communities and cross-sector partnerships.
Experience managing grant-funded initiatives.
Experience with implementation science, quality improvement, or organizational change management.

PHYSICAL REQUIREMENTS

Close visual acuity to perform activities such as preparation and analysis of documents; viewing a computer terminal; and extensive reading.

Physical activity in this position includes crouching, reaching, walking, talking, hearing and repetitive motion of hands, wrists, and fingers.
Sedentary work with lifting requirements up to ten pounds, sitting for extended periods of time.
Mental concentration is required in all aspects of work.
This work may require travel across the award site(s).

RESIDENCY REQUIREMENTS: The person in this position is required to reside in North Carolina or within 40 miles of the NC border.

SALARY: Depending on qualifications & experience of candidate. This position is exempt and is not eligible for overtime compensation.
