Full Job Description

JOB SUMMARY

The Intake Coordinator will act as the onsite specialist to determine program eligibility and the need for further services, while offering support to the Healthy Start Intake Assistants and Care Coordinators. This position will support program by completing initial contact interviews with pregnant women and parents of infants screened for or referred to Healthy Start. The Intake Coordinator will process pre- and post-natal risk screens and community referrals as well as provide outreach and case management/coordination services to eligible pregnant and postpartum women and their infants.

POSITION EXPECTATIONS AND RESPONSIBILITIES

Complete all data entry accurately and timely
Reschedule missed Healthy Start appointments
Must be non-judgmental and culturally sensitive when engaging with clients and project participants
Develop and maintain a comprehensive understanding of maternal/child health issues deemed risk factors during pregnancy and early childhood as well as associated helpful community resources
Develop and maintain an excellent working knowledge of the Well Family System and State of Florida Healthy Start Standards and Guidelines
Determine and designate “Program Eligibility” based on the information provided by the client during the initial contact interview and take appropriate actions (e.g., refer the case to the appropriate program or close the case) based on these findings
Initiate and complete initial client contact within the specified program timeframes and guidelines based on the prenatal screen, infant screen, or referral information
Manage a caseload while maintaining established quality and quantity standards
Provide referral information to educate and improve the client’s knowledge and understanding of available community resources and services that support the well-being of women during pregnancy or during the child’s infancy, (e.g., WIC, food banks, housing, mental health service, etc.)
Coordinates client referrals and interagency activities, including contacting other agencies to follow-up on referrals
Ensure referrals and closures have been appropriately linked and documented within the database system and agency data function
Ensure required efforts are made when attempting to reach clients, as stated in the program’s guidelines; this may include driving to the client’s home to make a final attempt to contact the client before closing the case due to unsuccessful attempts to reach the client via alternate methods
Prepare and submit appropriate administrative reports accurately and on time (e.g., caseload reports, timesheets, logs, etc.)
Follows up with patient on compliance with provided care plan
Properly organizes client discharge planning and case closure
Conducts regular telephone calls and face-to-face visits to client
Participate in training, supervision, and team meetings, as requested
Perform all other duties as assigned

EDUCATION AND EXPERIENCE

A bachelor’s degree or higher from an accredited college or university in human services, social sciences, social work, nursing, health education, health planning, healthcare administration, or related field with two (2) years of public health/community development experience. College transcripts will be required for the successful applicant.
Minimum of two (2) years of experience in human or social services, working with families and knowledge of referral and community resources processes
BLS CPR Certification required and maintained current

KNOWLEDGE, SKILLS, AND ABILITIES

Must have excellent written and verbal communication skills and the ability to communicate in an efficient, professional, and friendly manner
Professional communication, documentation, and time management skills
Ability to work under pressure and at a fast pace
Ability to multi-task
Critical thinking skills
Problem solving skills
Efficient and proficient in all computer applications under which work is assigned including email, Adobe®, Microsoft Office® programs, electronic medical records systems, and other programs as assigned
Ability to work well with people and interface effectively with a diverse population
Knowledge of methods of compiling, organizing, and analyzing data
Knowledge of HIPAA guidelines and maintain confidentiality of all information
Must have a valid Florida driver’s license, reliable transportation, and a good driving record
Demonstrate the ability to maintain control and evaluate what needs to be done to minimize the stresses of working under pressure and meeting deadlines
Ability to be flexible regarding work location and work schedule, occasionally including evenings or weekends outside of regular business hours, based on business needs.

PREFERRED QUALIFICATIONS

Bilingual in English and Spanish
Prior Healthy Start program experience is preferred but not required
Knowledge of referral and community resources processes

PHYSICAL REQUIREMENTS

Ability to stand, walk, or view a computer screen for extended periods of time
Must be able to sit or stand for an extended amount of time
Must be able to perform reaching, lifting, and bending motions
Frequent use of keyboard and telephone
Ability to lift up to 25 pounds
Ability to perform repetitive hand and wrist motions for extended periods of time
Ability to effectively talk and listen to patients

RELATIONSHIP REPORTING

Reports to the Program Manager and/or Director of Healthy Start

Healthy Start requires all new hires to pass a local and national Level 2 criminal background check before hiring.
