Title: Registered Nurse (RN) - Acute Care Case Manager / Discharge Planning

Facility: Providence Saint Mary Medical Center

Location: Apple Valley, CA

Contract: 13 Weeks

Schedule: Day Shift | 8:00 AM - 4:30 PM

Shift: 5 × 8-hour shifts

Job Summary

We are seeking an experienced Registered Nurse (RN) - Case Manager for a 13-week travel assignment in the Case Management department at an acute care hospital in Apple Valley, California.

The ideal candidate will have a minimum of 2 years of acute care case management/discharge planning experience, strong hospital experience, and demonstrated competency in care coordination, utilization review, discharge planning, and arranging post-acute services.

This position requires a seasoned travel nurse. First-time travelers will not be considered.

Required Qualifications

Active California RN license - Required.
A pending California RN license may be accepted for submission, but the nurse must be fully licensed by the start date.
Minimum 2 years of acute care case management/discharge planning experience – Required.
Acute-care hospital experience - Required.
Behavioral health experience alone does not meet the experience requirement.
Previous travel nursing experience - Required.
Must be a seasoned traveler; first-time travelers are not accepted.
Epic electronic medical record experience - Required.
Strong knowledge of hospital case management and discharge planning processes.
Ability to independently manage a caseload of up to approximately 25 patients.
Strong communication, organization, critical-thinking, and care-coordination skills.

Adoption and foster-care considerations
Complex family/social situations
Identification and coordination of appropriate community resources

Post-Acute Care Coordination

The RN Case Manager must be able to independently coordinate:

Durable Medical Equipment (DME)
Skilled Nursing Facility (SNF) placement
Ambulance and medical transportation
Appropriate post-acute services
Follow-up care and community resources

Preferred Clinical Background

Experience working with patients from multiple acute-care specialties is preferred, including:

Medical-Surgical
Emergency Department
ICU
Surgical Services

Patient Population

Neonates
Adolescents
Adults
Geriatric patients

Typical caseload: Up to 25 patients
Day ratio: Approximately 1:25
Weekend ratio: Approximately 1:45

Schedule & Weekend Requirement

5 × 8-hour shifts
8:00 AM – 4:30 PM
Day shift
Weekend rotation required based on department needs, approximately every other weekend
No regular floating expectation within the department
Holiday commitment may be required according to facility/travel policy

License & Certification Requirements

California RN license - Required.
Pending California license may be considered during submission, but an active California license is required before the assignment begins.
No additional specialty certification such as ACLS, PALS, or ONS chemotherapy certification is specified for this Case Manager position.
Candidates must meet all facility-specific credentialing requirements before starting.

Documentation & Technology

Epic experience - Required.
Computerized clinical documentation experience required.
Ability to accurately document assessments, utilization review, care coordination, discharge plans, and patient outcomes.

Travel Requirements

Seasoned travel nurse required.
First-time travelers are not accepted.
Previous travel assignment experience must be demonstrated.
Travelers may be required to float between applicable facilities within the state according to facility policy.
Local candidates within the applicable radius may be considered at the facility's local rate.

Pay: $65.00 - $68.00 per hour
