Full Job Description

Adult Day Health Care (ADHC) is seeking a Social Worker to join our team in San Bernardino, CA! This is an exciting opportunity for a motivated Social Worker who is looking for a career with a growing organization. We provide services to help individuals with developmental disabilities develop their social and emotional skills and are a valued part of the community. We are looking for someone who wants to make a difference in the lives of our clients and their family members.

Purpose

In order to provide help to participants and their families with personal, family and adjustment problems that interfere with the effectiveness of treatment, the Social Worker shall provide:

Responsibilities

Initial screening of referrals
Initial Assessment, quarterly, and reassessments to determine each participant's social and emotional factors affecting the health problem, and family's ability to cope with the situation.
Develop person centered treatment plan taking into consideration the health needs, the participants goals.
Liaison with the participant's family, home and other community agencies serving the participant.
Case management
Provide supervision for social work assistants or aides and define responsibilities.

Qualifications

Master's degree in social work from an accredited school of social work.
Holds a master's degree in social work psychology, gerontology, or counseling.
1 year of experience providing social services in one or more fields of aging, health, or long-term care services; or is licensed by California Board of Behavioral Sciences or bachelor's degree in social work from an accredited school with 2 years of experience providing social services in one or more of the fields aging, health, or long-term care services.
Bilingual (Spanish & English) required

Social work services are available during all hours of operation.

Job Type: Part time Saturdays (weekends)

Schedule/Hours: 8:00am-2:00pm

Expected hours:24-30 per month

Work Location: In person
