Now Hiring: Physical Therapist (PT) Home Health - PRN

Location: Arcadia and surrounding areas of Desoto County

Position Summary

The Physical Therapist provides skilled physical therapy services to patients in their homes under physician orders. This role is responsible for evaluating patients, developing individualized treatment plans, and delivering evidence-based care that improves function, promotes independence, and enhances quality of life.

Responsibilities

Perform comprehensive physical therapy evaluations and establish individualized plans of care.
Provide skilled physical therapy services in accordance with physician orders and agency policies.
Assess strength, balance, mobility, gait, endurance, and functional limitations.
Monitor patient progress and modify treatment plans as clinically appropriate.
Educate patients and caregivers on home exercise programs, safety, fall prevention, and adaptive equipment.
Document evaluations, treatments, and patient progress accurately and timely within the electronic medical record.
Communicate changes in patient condition with physicians and the interdisciplinary care team.
Supervise Physical Therapy Assistants (PTAs) in accordance with Florida regulations and agency policies.
Participate in case conferences, care coordination, and required agency education.

Qualifications

Graduate of an accredited Physical Therapy program.
Current Florida Physical Therapist (PT) license in good standing.
Minimum one (1) year of physical therapy experience required; home health experience preferred.
Valid Florida driver's license, reliable transportation, and current auto insurance.
Excellent communication, organization, and time management skills.
