Full Job Description

The Center for Health & Recovery (CHR) is seeking a skilled, compassionate, and highly organized Registered Nurse (RN) to join our Outpatient Clinic serving individuals living with Serious Mental Illness (SMI).

This position is ideal for an RN who thrives in a fast-paced, community-based behavioral health setting and understands the clinical complexity of individuals with co-occurring mental health and substance use conditions.

Position Responsibilities

The RN will function as a core member of the Behavioral Health Home ( BHH) Team and will be responsible for:

Clinical & Nursing Duties

Manage and monitor a shared BHH caseload
Conduct brief nursing assessments and health status evaluations
Administer long-acting injectable medications
Perform blood draws and coordinate lab services
Monitor medication adherence and effectiveness
Oversee medication ordering, tracking, storage, and disposal in accordance with protocols as it applies
Provide medication education to members and families
Complete nursing documentation
Monitor vital signs and identify changes in medical or psychiatric condition
Conduct clinical triage and determine emergent vs. non-emergent needs
Escalate concerns appropriately to prescriber/provider per protocol

Care Coordination

Coordinate with psychiatric providers, primary care providers, pharmacies, hospitals, and community partners
Participate in daily team meetings and shared treatment planning
Assist in transitions of care following hospitalization
Coordinate and document communication with health care providers for hosptialization discharges
Support integrated care for members with co-occurring substance use disorders

Compliance & Documentation

Maintain timely, accurate, and thorough clinical documentation
Adhere to Arizona Nurse Practice Act and professional scope of practice
Ensure proper chain of command and escalation procedures are followed

Qualifications

Active Arizona RN license (in good standing)
Experience in behavioral health or psychiatric nursing preferred
Experience with SMI population strongly preferred
Knowledge of psychotropic medications and side-effect management
Experience administering injectable medications
Experience with blood draws and lab coordination
Strong clinical judgment and triage skills
Ability to manage workload in a community-based setting
Excellent documentation and communication skills
Those who identify as having their own lived experience are highly encouraged to apply

Preferred Experience

Experience working with individuals with co-occurring substance use disorders
Familiarity with AHCCCS/Medicaid behavioral health systems
Experience in crisis intervention and de-escalation
