Position Overview
We are seeking a compassionate, clinically minded Scottsdale-based representative to join our growing team. You will work directly with seniors and their families—often during moments of crisis or transition—to assess needs, educate them on options, and facilitate placements into the most appropriate care settings.
Ideal candidates bring a background as a social worker or Registered Nurse (RN) or 2 years of clinical or hospital experience. Ideal candidates will have a strong background in health care or with experience in hospice, home health, hospital discharge, case management, or similar senior/healthcare settings. You will serve as a trusted guide, combining medical understanding with strong relationship and communication skills.
- Conduct thorough in-person or virtual assessments of seniors’ medical, functional, cognitive, social, and financial needs.
- Educate families on care continuum options (independent living, assisted living, memory care, in-home care, skilled nursing/rehab, etc.) and help them evaluate communities against clinical and personal criteria.
- Guide families through hospital or rehab discharge planning and transitions.
- Build and maintain referral relationships with hospitals, physicians, hospice agencies, home health providers, elder-law attorneys, fiduciaries, and other community partners in the Scottsdale/East Valley area.
- Coordinate tours, compare communities, and support seamless move-ins and ongoing follow-up.
- Maintain accurate case notes and communicate progress with the internal team.
- Provide ongoing support as care needs evolve—Senior Care AZ stays with families beyond the initial placement.
- Represent the company with professionalism, empathy, and integrity at all times.
Qualifications
- Bachelor’s degree or higher in Social Work, Nursing, or a related healthcare field (LMSW/LCSW or RN strongly preferred but not mandatory).
- Experience in hospice, palliative care, home health, hospital case management/discharge planning, geriatric care management, or senior living placement.
- Strong clinical assessment skills and the ability to recognize appropriate levels of care.
- Excellent interpersonal, listening, and communication skills—able to build trust quickly with families in stressful situations.
- Valid Arizona driver’s license, reliable transportation, and willingness to travel throughout the Scottsdale and greater Phoenix metro area for assessments, tours, and relationship-building.
- Comfort with technology (CRM, scheduling, basic documentation).
- Compassionate, patient, organized, and solutions-oriented mindset.
Preferred
- Hospice, palliative, or end-of-life care experience.
- Knowledge of Arizona senior living communities, Medicare/Medicaid, VA benefits, and long-term care funding options.
- Bilingual (Spanish/English) a plus.
- Existing professional network in Scottsdale healthcare or senior services.
