Mental Health Professional II - Adult IOP

Falls Church, VA

Position Type: Full-Time

Shift: Day Shift | Tuesday, Wednesday, Thursday 12 noon-8p, Fridays 9-5

Salary Scale: $31.10 - $43.54

Dominion Hospital, the only freestanding mental health facility in Northern VA, is seeking an experienced Mental Health Professional II - Adult IOP to support our Intensive Outpatient Program. In this key role, you will be responsible for high quality, patient-centered care by providing supervised mental health services to patients and their families. The Mental Health Professional II - Adult IOP performs assessments, psycho-education, process groups, treatment planning, discharge planning and therapeutic counseling support through evidenced based interventions in individual, family and group therapy.

What You'll Do

Conducts individual, group and family psycho-educational and process groups.
Develops treatment plans and attends treatment planning meetings and reviews on an ongoing basis.
Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition.
Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
Completes assessments, group notes and all other documentation promptly and in compliance with requirements, as appropriate to scope.
Collaborates with the multidisciplinary team to assess and coordinate patient’s discharge planning needs and readiness for discharge.
Conducts post-discharge follow-up as indicated.
Evaluates re-admissions for gaps in discharge planning processes. Formulates a plan and implements new strategies.
Develops and maintains a community resource guide including access to public assistance/alternatives.
Facilitates and participates in Community Education, Activities and Support Groups, as appropriate to scope.
Contributes to ensuring a safe, structured milieu with evidence-based treatment modalities/resources.
Identifies cultural considerations, gender identity and preferred language for patients and family.
Demonstrates appropriate application of age/developmental stage specific standards, policies and procedures, and guidelines in assessing and treating patients to ensure patient safety.
Maintains active and timely communication with patients, families, multidisciplinary team and continuum of care providers.
Maintains professionalism and uses good communication skills in interacting with peers, staff, patients, families and in resolving conflicts.
Demonstrates understanding of and ability to maintain clear professional boundaries with peers, patients and families.
Understands state commitment processes, abuse reporting requirements, duty to warn and guardianship processes.
Demonstrates knowledge and role in hospital codes.
Serves as an educational resource to patients, families, interdisciplinary team members, payer representatives, and other parties as appropriate regarding behavioral services.
Reviews, coaches, and assists with training of new staff/interns as assigned within approved practice and program guidelines.
Participates in developing department goals/objectives and clinical programming.
Performs utilization review as assigned.
Assists with Process and Quality Improvement initiatives.
Performs other duties as assigned.

Qualifications

Education: Master’s Degree in Social Work or related field-Required

Licensure: Currently registered as a Licensed Master in Social Work (LMSW) in the state(s) of practice, according to law and regulation.-Preferred. Currently licensed as a Licensed Professional Counselor Associate (LPCA)-Preferred

Certifications: Basic Life Support for Healthcare Providers required and Crisis Prevention Institute required as a part of the general hospital orientation.

Experience: Experience in Mental Health or related field-Preferred
