Job Title: Patient Health Navigator (Bilingual preferred in English/Spanish)

Position Type: Full-Time, Exempt

Position Summary:

The Patient Health Navigator (Bilingual English/Spanish) is a key member of the outreach and care coordination team at Washington Health Institute (WHI). This position supports the agency’s Outreach and Rapid Linkage to Care program, guided by the Hi-V model (Find, Test, Teach, Link, and Keep).

This role requires a Master’s or Bachelor’s degree in Public Health, Social Work (MPH/BPH, MSW/BSW), or a related field, and/or a minimum of one (1) year of experience working with individuals living with HIV and/or other high-risk or underserved populations.

The Patient Health Navigator will provide outreach, sexual health education, HIV/STI/Hepatitis C testing and counseling, linkage to care, and retention support. The role is designed to reduce barriers to care and improve health outcomes for vulnerable populations impacted by HIV, STIs, and viral hepatitis in DC, MD and VA.

Primary Responsibilities:

Collaborate with patients, clinicians, social workers, and care teams to establish individualized care goals.
Provide patient navigation services addressing social determinants of health (housing, transportation, food insecurity, employment, mental health, and substance use).
Assist patients with insurance enrollment, eligibility, and access to appropriate coverage resources.
Support implementation of HIV program objectives aligned with Ryan White Part A Early Intervention Services (EIS) and Syndemic workplans and scope of services.
Develop and maintain relationships with community-based organizations and outreach partners.
Coordinate and participate in outreach events, community engagement activities, and testing initiatives.
Conduct rapid HIV, Hepatitis C, and STI testing in both clinical and community-based settings.
Provide counseling, linkage to care, and follow-up support for newly diagnosed HIV-positive individuals and individuals eligible for PrEP.
Facilitate referrals and navigation to medical, behavioral health, housing, legal, employment, and social support services.
Complete Ryan White eligibility determinations at intake and annually.
Develop Harm Risk Reduction Plans (HRRP) with patients to support individualized health and stability goals.
Maintain ongoing patient contact to support treatment adherence, retention in care, and barrier reduction.
Provide crisis intervention and emergency assistance referrals, including food, transportation, and utility support.
Participate in case conferencing, medical rounds, and interdisciplinary care coordination at least twice per year per client.
Ensure effective collaboration with internal and external providers while maintaining confidentiality standards.
Monitor patient engagement and follow-through with referred services.
Maintain accurate and timely documentation in Electronic Medical Records (EMR) and CAREWare.
Support monthly, quarterly, and annual reporting requirements, including retention and performance reporting.
Perform other duties as assigned.

Skills and Qualifications:

Bachelor’s or Master’s degree in Public Health, Social Work, or related field required.
Minimum of one (1) year experience in outreach, case management, or patient navigation with individuals affected by HIV or at risk for HIV/STIs.
Strong verbal and written communication skills in English and Spanish required.
Proficiency in Microsoft Office Suite and electronic data systems (EMR, CAREWare preferred).
Strong organizational skills with the ability to multitask and manage competing priorities.
Demonstrated ability to work effectively in team-based, mission-driven environments.
Commitment to professionalism, confidentiality, patient safety, and quality care standards.

Clinical & Professional Requirements:

Experience working with diverse and underserved populations, including BIPOC communities, LGBTQIA+ individuals, transgender and gender nonconforming individuals, individuals experiencing homelessness or housing instability, and individuals living with HIV.
Ability to work onsite Monday–Friday, 8:00 AM – 4:30 PM, with flexibility for evening and weekend community outreach events as needed.
Compliance with organizational vaccination requirements, including Hepatitis B, influenza, and COVID-19 vaccines.
Ability to exercise independent judgment while knowing when to escalate clinical or client-related concerns utilzing a red-carpet established process.
Strong interpersonal skills with the ability to communicate effectively and compassionately with patients, families, and care teams.
Ability to manage ambiguity using structured problem-solving approaches.
Strong work ethic grounded in accountability, teamwork, and initiative.
Maintain up-to-date client records and documentation in compliance with agency standards and grant requirements.
Support data quality and reporting efforts to ensure program accountability and performance tracking.
Demonstrate cultural humility and sensitivity in all client interactions.
