Hospital Liaison

About the Role:

Serenity Estates, a nurse-owned and resident-focused organization, is seeking a compassionate and highly organized Hospital Liaison to join our team! As a Hospital Liaison, you will play a vital role in ensuring seamless transitions between our assisted living facility and the hospital. Our commitment to providing exceptional care and compassion is unwavering, and we are looking for a dedicated professional to join our mission.

Responsibilities:

Coordinate with hospital staff to ensure efficient and effective communication regarding resident hospitalizations, discharges, and transfers
Conduct thorough assessments of resident needs and develop personalized care plans in collaboration with our interdisciplinary team
Advocate for residents' rights, dignity, and well-being, promoting a culture of respect and empathy
Facilitate communication between residents, families, and healthcare providers to ensure continuity of care
Participate in quality improvement initiatives to enhance the overall resident experience
Build and maintain strong relationships with hospital staff, physicians, and other healthcare professionals
$80,000 to 100,00K depending on education and experience.
