Halo Hospice is a privately owned and operated Hospice company in Fort Worth, Tx. We are looking for the best of the best to join our team!

Currently searching for a RN on call that would work 7 on and 7 off! Please let us know if you are interested in joining a fast paced and growing company!

Job Summary
We are seeking a compassionate and dedicated Hospice Nurse to provide high-quality care to patients in their homes or hospice facilities. The ideal candidate will be responsible for delivering patient-centered care, managing symptoms, and ensuring comfort for patients with terminal illnesses. This role requires strong clinical skills, empathy, and the ability to work collaboratively with families and healthcare teams.

Responsibilities

Conduct comprehensive physical examinations and assessments of patients' health status.
Develop and implement individualized care plans based on patient needs and preferences.
Administer medications and treatments as prescribed, utilizing aseptic techniques.
Provide emotional support and education to patients and their families regarding end-of-life care options.
Coordinate with interdisciplinary teams for effective case management and continuity of care.
Monitor vital signs and manage symptoms such as pain, nausea, and respiratory distress.
Assist in ventilating patients when necessary and provide emergency interventions as required.
Document all patient interactions accurately in medical records.
Participate in team meetings to discuss patient progress and adjust care plans accordingly.

Skills

Proficient in aseptic technique to prevent infection during procedures.
Experience in medical office settings or clinics is preferred.
Ability to perform physical examinations effectively.
Familiarity with home care practices and hospice services.
Knowledge of case management principles to coordinate comprehensive patient care.
Experience in emergency room settings is a plus for handling acute situations.
Skills in administering treatments such as botulinum toxin treatment when applicable.
Competence in performing sutures for minor wounds or injuries.
