We are looking for an experienced, compassionate, and motivated Resident Care Coordinator who enjoys being part of a hands-on leadership team. This is a working leadership position where you'll spend time both coordinating resident care and supporting the team on the floor. No two days are the same, and we are looking for someone who enjoys problem-solving, mentoring staff, and helping our residents live their best lives.

Schedule
- Approximately 3-4 days per week
- Primary schedule is Sunday–Wednesday
- Additional hours are often available to help cover vacations, call-offs, and staffing needs.
- We understand life happens and are willing to be flexible with the schedule for the right candidate.
What You'll Do
As our Resident Care Coordinator, you'll help oversee the daily care of our residents while working closely with caregivers, medication technicians, families, physicians, hospice, home health, pharmacies, and our nursing team.
Responsibilities include:
- Develop, update, and oversee individualized resident care plans.
- Complete resident assessments and monitor changes in condition.
- Communicate with families regarding resident status and care needs.
- Coordinate with physicians, hospice, home health, therapists, pharmacies, and outside providers.
- Assist with new admissions, move-ins, hospital returns, and discharges.
- Ensure resident documentation is accurate, complete, and compliant with Oregon regulations.
- Participate in incident investigations, follow-up documentation, and implementation of interventions.
- Conduct routine resident rounds to ensure quality care and identify concerns before they become problems.
- Coordinate appointments, transportation, and outside services.
- Support, mentor, and train caregivers and medication technicians.
- Assist with staffing support and daily operations.
- Help maintain compliance with state regulations and company policies.
- Promote a positive, respectful, and team-oriented culture.
This is a Working Leadership Position
Because we are a small memory care community, our leaders work alongside the team. The Resident Care Coordinator is expected to assist with direct resident care, medication support (if qualified), resident supervision, and floor coverage whenever needed. We believe the best leaders are visible, approachable, and willing to jump in wherever they're needed to support residents and staff.
What We're Looking For
The ideal candidate is someone who:
- Has previous experience in a Resident Care Coordinator, Lead Caregiver, Medication Technician, or similar leadership role.
- Has experience working in memory care or with individuals living with dementia.
- Understands person-centered dementia care and enjoys building relationships with residents.
- Is organized, dependable, and able to prioritize multiple responsibilities.
- Communicates professionally with families, providers, and coworkers.
- Enjoys mentoring, coaching, and supporting team members.
- Is confident making decisions while also knowing when to seek guidance.
- Works well independently without constant supervision.
- Has strong documentation and computer skills.
- Maintains professionalism, confidentiality, and compassion in every interaction.
- Holds CPR/First Aid certification or is willing to obtain it.
- Has knowledge of Oregon Assisted Living and Memory Care regulations (preferred).

Pay: $20.00 - $21.00 per hour
