Full Job Description

We are seeking an experienced and compassionate Registered Nurse (RN) Supervisor to join our Certified Home Health Agency (CHHA) team, supporting our Women's Health and Pediatrics Program. In this leadership role, you will oversee and support field nurses while ensuring the delivery of high-quality, patient-centered care to women and pediatric patients in the home setting.

We will also consider an experienced home care field nurse who is looking to transition into a supervisory position. If you have strong clinical skills, enjoy mentoring others, and are ready to take the next step in your career, we'd love to hear from you.

Key Responsibilities

Clinical Oversight & Patient Care

Provide clinical supervision and guidance to field nurses caring for Women's Health and Pediatric patients.
Conduct patient assessments, including admissions and supervisory visits.
Ensure individualized plans of care are appropriate, updated, and followed.
Monitor patient progress and intervene as needed to ensure optimal outcomes.
Collaborate with physicians and interdisciplinary team members to coordinate quality care.

Staff Supervision & Support

Supervise, mentor, and support RN field staff.
Conduct performance evaluations and provide ongoing coaching and feedback.
Assist with orientation and training of new nursing staff.
Serve as a clinical resource for Women's Health and Pediatric cases.
Foster a positive, collaborative, and patient-focused work environment.

Care Coordination

Coordinate care with physicians, therapists, home health aides, and other interdisciplinary team members.
Participate in case conferences and care planning meetings.
Ensure services are delivered in accordance with physician orders and agency standards.

Documentation & Compliance

Review and ensure the accuracy of OASIS documentation and clinical records.
Maintain compliance with all federal, state, and agency regulations.
Oversee timely completion of required documentation and quality assurance activities.

On-Call Responsibilities

Participate in rotating on-call coverage.
Provide clinical guidance and support to field staff during on-call hours.

Qualifications

Active Registered Nurse (RN) license in New York State.
Experience in Women's Health, Pediatrics, Home Health, or a related clinical specialty required.
Minimum of 2 years of nursing experience required.
Previous supervisory or leadership experience preferred, but experienced home care field nurses looking to advance into leadership are encouraged to apply.
CHHA experience is a plus.
Knowledge of OASIS and home health regulations preferred.
Excellent clinical judgment, communication, organizational, and leadership skills.
Valid driver's license and ability to travel throughout Brooklyn and Queens.

Physical Requirements

Ability to travel to patient homes, including walk-ups and varied home environments.
Ability to stand, walk, bend, and lift as needed throughout the workday.
Ability to lift up to 50 pounds independently and more with assistance when necessary.

Health Requirements

Completion of pre-employment and annual health assessments.
Compliance with all New York State immunization and health requirements.
