Job Summary:
Under close supervision and within established systems, schedules, and protocols receives and input information from the Minimum Data Set (MDS).
Essential Duties (include but are not limited to):

Enters information from the MDS document into the MDI system by mandated, established, and expected deadlines, incorrect record format, and with a high degree of accuracy.

Compares data entered against the original MDS document; makes corrections as needed.

Collects MDS documents within mandated, established, and expected time frames.

Retrieves and prints initial feedback reports and final validation reports; locks MDS after final validation is received.

Notifies appropriate coworkers of any questions related to the MDS data, delays in data entry, system problems, or other obstacles that may arise in the course of work.
Requirements:

Current LVN license

Min. 1 year as MDS Coordinator in LTC setting: 1 year

MDS Certified preferred

Proficient in MDS 3.0 reimbursements.
