Job Overview
We are seeking a dedicated and highly skilled RN Assessment Nurse to join our dynamic healthcare team. In this vital role, you will be responsible for conducting in-home comprehensive patient assessments to determine appropriate care plans, facilitate and coordinate with multidisciplinary teams. Your role will ensure patients receive the highest quality of personalized care. This position offers an exciting opportunity to make a meaningful impact on patient outcomes through thorough evaluation and compassionate support.

Duties

Perform detailed patient Start of Care assessments and re-certification assessments including vital signs, fall risk, and home saftey assessments.
Collaborate with case managers, and other healthcare professionals to develop and update individualized treatment plans.
Utilize clinical knowledge in specialties such as geriatrics, behavioral health, and infectious disease care to provide expert assessments.
Support utilization management efforts by reviewing cases for appropriateness of services and resource allocation.

Skills

Excellent patient service skills with a focus on compassionate patient assessment and case management.
Ability to interpret vital signs accurately and perform comprehensive patient evaluations across diverse clinical scenarios.
Familiarity with medical records management, discharge planning processes.
