Position: Intensive Support Coordinator

Reports to: Behavioral Health Clinical Director

Summary:

The Intensive Support Coordinator is a Certified Peer Specialist (CPS) or Certified Addiction Recovery Empowerment Specialist (CARES) who will provide peer support, advocacy, and mentoring to individuals experiencing mental health and/or substance use challenges within the Permanent Supportive Housing (PSH) program. In addition to peer support, the CPS/CARES will assist with case management tasks to ensure that clients receive comprehensive care. The CPS/CARES will work closely with the Behavioral Health Clinician to help clients achieve housing stability and recovery goals.

Primary Responsibilities:

Peer Support & Mentorship

Provide one-on-one peer support and mentorship to clients, helping them set recovery and housing stability goals.
Share lived experiences with mental health and/or substance use recovery to inspire hope and demonstrate the possibilities of recovery.
Facilitate peer-led groups focused on recovery, wellness, life skills, and housing stability.
Offer support in navigating community resources, healthcare, and housing services.

Advocacy & Empowerment

Empower clients to advocate for themselves in service and treatment planning meetings.
Assist clients in understanding and exercising their rights within housing and behavioral health services.
Accompany clients to appointments or meetings as needed, offering support and helping them navigate challenging situations.
Act as a liaison between clients and the on-site clinician to ensure clients’ voices are heard.

Recovery & Wellness Planning

Assist clients in developing and implementing personal recovery plans that align with their behavioral health and housing stability goals.

Promote wellness practices, including relapse prevention strategies, self-care, and coping mechanisms, tailored to individual needs.
Work collaboratively with the on-site clinician to provide input on treatment plans based on peer experience and client needs.

Case Management Support

Assist the on-site clinician in conducting needs assessments to identify gaps in services and areas where additional support is required.
Help clients connect to community resources, including healthcare, benefits programs, job training, and other supportive services.
Coordinate with the on-site clinician to follow up on service referrals and ensure that clients receive timely access to needed resources.
Support clients in completing necessary paperwork for housing, benefits, or health services, providing guidance and encouragement through the process.
Work with the on-site clinician to ensure that clients adhere to housing agreements and are connected to appropriate support services to maintain housing stability.

Crisis Support & Intervention

Provide emotional support during crises, helping clients stabilize and connect to immediate services as needed.
Collaborate with the on-site clinician during crisis situations, ensuring that clients receive appropriate interventions.
Assist in de-escalating situations and offering peer-level problem-solving approaches.

Documentation & Compliance

Maintain accurate and timely documentation of peer support activities, recovery planning, case management tasks, and client interactions, in accordance with agency standards.
Ensure compliance with all applicable peer support certification guidelines and ethical standards.

Professional Development

Participate in ongoing training to stay current with peer support best practices, trauma- informed care, case management, and housing-first principles.
Engage in regular supervision with the on-site clinician and/or Clinical Director to discuss client progress, challenges, and professional growth.

Qualifications:

High school diploma or equivalent required, bachelor’s degree preferred.
Certification as a Certified Peer Specialist (CPS) through the Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD) or as a Certified Addiction Recovery Empowerment Specialist (CARES) is required.
Lived experience with housing instability, mental health recovery or addiction recovery is required.
Demonstrated knowledge of the principles of peer support, recovery-oriented systems of care, case management, and harm reduction.
Previous experience working with individuals facing homelessness, housing instability, or behavioral health challenges is preferred.
Ability to establish rapport and build supportive, trusting relationships with clients.
Strong interpersonal communication skills and the ability to work collaboratively within a multidisciplinary team.
Basic computer skills, including proficiency with Microsoft Office and willingness to use electronic documentation systems.

Knowledge, Skills & Abilities:

Strong understanding of mental health and addiction recovery from a peer perspective, including wellness strategies, recovery supports, and self-advocacy.
Knowledge of case management processes, including service coordination and resource navigation.
Ability to relate to and support clients using trauma-informed and person-centered approaches.
Excellent problem-solving and conflict-resolution skills, with a focus on client empowerment and self-determination.
Willingness to share personal recovery experiences as a tool for helping others.
Commitment to the mission of HOPE Atlanta and to supporting individuals in achieving housing stability and personal recovery.
Ability to work effectively with diverse populations, including individuals from various cultural, ethnic, and socioeconomic backgrounds.
Proficiency in electronic health record (EHR) systems, MS Office Suite, Homeless Management Information System Client Track, and database systems, etc.
Possess a valid Georgia driver’s license, with a good driving record and reliable transportation, required.
