Full Job Description

Jewish HomeLife is looking for a MDS Coordinator to join our Skilled Nursing division, The William Breman Jewish Home! $6,000 Sign-On Bonus available!!!*

Summary: The MDS Coordinator is a registered nurse or certified in MDS responsible for coordinating the comprehensive assessment of residents. The MDS Coordinator reports to the Executive Director of the Skilled Nursing department. Although the MDS Coordinator primarily works the day shift, they are responsible for coordinating the written plan of care with all shifts and disciplines.

Qualifications:

Registered Nurse with a current State of Georgia license.
Minimum of two-years long term care experience.
Minimum of one-year MDS experience, MDS certification preferred.
Minimum of one-year Case Mix experience .
Demonstrate leadership and teaching abilities.
Demonstrate interviewing and clinical skills.

Responsibilities:

Ensures compliance with policy and procedures for meeting the federal guidelines of Resident Assessment Instrument .
Complete MDS to initiate the process that plan, organize, and evaluates nursing care according to established policies and procedures.
Assesses and reassesses residents as required by federal and/or state regulations and as needed to develop a nursing care plan for each resident.
Publishes a monthly calendar or schedule assessments as required by federal and/or state regulations for team members.
Confers with clinic and therapy staff, to develop, implement and evaluate care plans.
Schedules care conferences with residents and family members.
Staffs resident/client care conference (RCC).
Transmits required assessments to CMS.
Reviews and analyzes data generated from MDS transmittals to assist in managing quality measures.
Collaborates with Business office regarding billing as it relates to the MDS assessment.
Maintains certification and re-certification for medicare residents/clients.
Assists in rehab client discharge planning, notification and securing signatures of the Notice of Medicare Non-Coverage (NOMNC).
Attends daily update or as scheduled.
Attends all federally mandated staff meetings.
Continuous education to remain current with RAI/MDS and management of data collected.
Other duties as delegated by the Director of Health Services.

Supervisory Responsibility: No supervisory responsibility.

Physical Requirements: Ability to lift up to 30 pounds.

Mental Requirements: Ability to focus for extensive period of time.

*FT/PT Only. Paid incrementally over 1-year of employment.

Job Type: Full-time

Pay: $90,000.00 - $95,000.00 per year
