EMIC Healthcare is seeking an experienced, dependable Clinical Field Nurse (LPN) to support our growing home care organization.

This position is ideal for a nurse who enjoys working independently, traveling throughout Georgia, interacting with families, and helping ensure high-quality care is being delivered in the home.

The Clinical Field Nurse will work closely with EMIC Healthcare’s Director of Nursing and will primarily conduct scheduled in-home client visits, clinical assessments, supervisory visits, documentation reviews, and follow-up.

Schedule & Travel

This is a field-based nursing position.

The nurse will travel to assigned client homes throughout the workweek.

Each active client will generally receive an in-home visit approximately once per week based on clinical and program requirements.

Travel assignments will generally be scheduled within a maximum approximately 2-hour driving radius.

A company vehicle will be provided for authorized work-related travel.

The position requires regular driving and the ability to independently manage an assigned weekly client-visit schedule.

Responsibilities

Conduct scheduled in-home client visits
Complete nursing assessments and clinical observations
Perform supervisory and quality-assurance visits as assigned
Monitor client condition and identify changes or concerns
Review the client’s current plan of care and ensure services are being delivered appropriately
Review nursing documentation for completeness and accuracy
Evaluate the home care environment and identify potential safety or care concerns
Communicate with clients, parents, guardians, caregivers, and nursing staff
Provide education and support to families when appropriate
Identify documentation, staffing, clinical, or compliance concerns and promptly report them to the Director of Nursing
Assist with admissions, reassessments, recertifications, and other clinical visits as assigned
Ensure required clinical documentation is completed accurately and submitted timely
Maintain professional communication with field nurses and caregivers
Collaborate closely with the Director of Nursing and clinical team
Assist with maintaining compliance with EMIC Healthcare policies and applicable program requirements

Qualifications

Current and unrestricted Georgia nursing license
Minimum 1 year of nursing experience preferred
Home health, pediatric, private duty nursing, or community-based nursing experience strongly preferred
Pediatric nursing experience is a plus
Experience with medically complex clients, including G-tubes, tracheostomies, ventilators, feeding pumps, oxygen, and other skilled nursing needs is highly desirable
Strong clinical assessment skills
Excellent documentation and communication skills
Ability to work independently in the field
Excellent time-management and organizational skills
Comfortable traveling regularly between client homes
Valid driver’s license and acceptable driving record required for use of company vehicle
Ability to maintain confidentiality and comply with HIPAA requirements
