Full Job Description

Job Summary

We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA)/Personal Care Assistants (PCA) to join our dynamic home care team. In this vital role, you will provide high-quality, patient-centered care to individuals in home care environments. Your energy and enthusiasm will help foster a supportive atmosphere that promotes dignity, comfort, and well-being for all patients. The ideal candidate is proactive, detail-oriented, and passionate about making a positive difference in people’s lives through attentive caregiving and skilled assistance.

Duties

Assist patients with daily activities such as bathing, dressing, grooming, dental care, and mobility to promote independence and comfort.
Monitor vital signs including blood pressure, pulse, temperature, and respiration rates.
Support medication administration under supervision, ensuring proper handling and adherence to HIPAA regulations to maintain patient confidentiality.
Use specialized equipment such as Hoyer lifts and transfer methods to safely move patients while practicing manual handling and aseptic techniques.
Observe and report changes in patient conditions promptly to nursing staff; participate in care planning and documentation processes.
Provide emotional support and effective communication tailored to patients’ needs, including those with dementia or developmental disabilities.
Maintain a safe environment by following infection control protocols, practicing basic life support (BLS), and adhering to hospital or facility safety standards.

Requirements

Successful completion of a nurse aide training and competency evaluation program pursuant to the requirements of 42 CFR part 483, Subpart D, as revised or recodified, if applicable; or
Successful completion of a competency examination for Nurse aides recognized by the department; or
Successful completion of a Healthcare or personal care credentialing program recognized and approved by the department; or
Successful completion or progress in the completion of a 40-hour training program provided by a private home care provider, which addresses at least the following areas: Ambulation and transfer of clients, including positioning;
Assistance with bathing, toileting, grooming, shaving, dental care, dressing, and eating;
Basic first aid CPR;
Caring for clients with special conditions and needs so long as the services are within the scope of the tasks authorized to be performed by demonstration;
Home management;
Home safety and sanitation;
Infection control in the home;
Medically related activities to include the taking of vital signs; and
Proper nutrition
Completion of the orientation training
Signed “Statement of No Misconduct”
Negative TB test
Ability to read and write English
Ability to follow instructions
Provide the agency with true statements regarding qualifications requirements
