Job Description

Housing Support Program (HSP) Case Manager – Rehabilitation Housing

HSP Case Manager-10am-6pm-PT-(EOW)

General: The Housing Support Program (HSP) Case Manager works under the supervision of the Assistant Director or the HSP Clinical Supervisor depending on site assignment within the Rehabilitation Housing Program. The Case Manager is responsible for delivering individualized housing support services to adults living with severe and persistent mental illness (SPMI) who are enrolled in the Housing Support Program (HSP). This role focuses on sustaining permanent supportive housing, preventing eviction, and promoting recovery, independence, and community integration.

The Case Manager provides direct services consistent with the Housing First philosophy, Psychiatric Rehabilitation (PSR) principles, and CFI’s mission. Services are designed to ensure long-term housing stability and wellness while respecting individual choice and supporting resiliency.

I. Major Duties and Responsibilities:

A. Housing Stability Supports

1. Assist individuals with housing search, leasing, and move-in processes.

2. Educate individuals on tenant rights and responsibilities, landlord expectations, and lease requirements.

3. Conduct safety and wellness checks, including housing safety inspections.

4. Develop and monitor Housing Stability Support Plans as an adjunct to each individual’s IRP.

5. Provide early intervention when housing is at risk (e.g., late rent, lease violations, conflicts with landlords/neighbors).

6. Support individuals with annual housing recertification, inspections, and federal/state housing program requirements.

7. Coach and model effective communication with landlords, property managers, and neighbors.

8. Serve as the first point of contact for landlords/property owners regarding tenant concerns.

B. Recovery and Community Integration Supports

1. Provide skills training in independent living (e.g., budgeting, home management, transportation, meal planning).

2. Link individuals to natural supports, community resources, and recovery services.

3. Assist with applications for public benefits (SSI/SSDI, SNAP, Medicaid, WIC) and referrals to employment/education supports.

4. Facilitate social, educational, and recovery-oriented activities that promote wellness and community engagement.

5. Support medication self-administration monitoring in alignment with CFI policy.

6. Encourage participation in tenant association meetings and community integration opportunities.

C. Case Management & Documentation

1. Maintain ongoing contact with assigned caseload to support housing stability and recovery goals.

2. Document all services in the electronic health record (EHR) in compliance with DBHDD, Medicaid, and CFI standards.

3. Complete progress notes that clearly reflect interventions, outcomes, and next steps.

4. Complete timely incident reports, crisis response documentation, and follow-up notes as required.

5. Participate in interdisciplinary treatment team meetings, case reviews, and housing stability reviews.

D. Collaboration & Program Support

1. Coordinate with landlords, housing authorities, DBHDD, and community partners to ensure successful tenancy.

2. Engage in landlord recruitment, relationship-building, and dispute resolution.

3. Assist with program compliance, CARF preparation, and audits.

4. Participate in ongoing professional development related to Housing First, PSR, and recovery practices.

5. Conduct routine property inspections, submit maintenance requests, and support health and safety compliance.

II. Competencies, Knowledge, Skills, Abilities

a. Knowledge of Housing First, permanent supportive housing, psychiatric rehabilitation, and recovery-oriented services.

b. Understanding of federal and state housing programs (GHVP, Housing Choice Voucher, HUD 811, etc.).

c. Strong skills in crisis prevention, early intervention, and eviction prevention.

d. Excellent communication and interpersonal skills for engaging residents, landlords, and partners.

e. Ability to provide trauma-informed, culturally responsive, and person-centered care.

f. Effective organization, documentation, and case management skills.

g. Capacity to work independently and as part of a team in a community-based environment.

III. Qualifications

a. Minimum of two years’ experience working with adults diagnosed with SPMI OR two years of college in psychology, social work, human services, or related field.

b. Experience in housing supports, case management, or community mental health preferred.

c. Current Georgia driver’s license, insurable driving record, and acceptable Motor Vehicle Report (MVR).

d. Basic computer skills; CareLogic experience preferred.

e. Ability to work evenings, weekends, and holidays as needed.

F. Physical Requirements:

Must be able to lift 25 – 50lbs, retrieving and placing materials on high shelves and on floor-level shelves; standing, walking; carrying; grasping; hearing; seeing; sitting; kneeling; climbing; balancing; stooping; crouching and reaching.
