About Us
The South Jersey AIDS Alliance is a caring, compassionate organization dedicated to the fight against HIV/AIDS, aiming to improve health outcomes and reduce barriers to care for individuals and families in our community. We provide client-centered services that promote dignity, equity, and well-being.

About the Role
The Status Neutral Case Manager plays an important role in supporting individuals living with or at risk of HIV by providing comprehensive, client-centered case management and care coordination services. This position works closely with clients to reduce barriers to care, connect individuals with medical, behavioral health, legal, and social support services, and promote long-term health and well-being. The Status Neutral Case Manager collaborates with healthcare providers, insurance carriers, and community partners to ensure clients receive the services they need. This role also supports HIV testing and prevention efforts by assisting individuals at risk of HIV infection with education and access to testing services and prevention tools such as Pre-Exposure Prophylaxis (PrEP.)

· Target Start Date for this position is: June 2026

· Role may include occasional evening/weekend shifts

What You’ll Do

Build trusting, respectful relationships with clients, community partners, and colleagues to support engagement and reduce barriers to care.
Conduct outreach in community settings to identify and engage individuals who may benefit from case management services.
Provide client-centered case management and care coordination for individuals living with or at risk of HIV.
Conduct HIV testing and provide information on HIV prevention and testing services.
Assess clients for PrEP eligibility and support individuals in initiating and maintaining a PrEP regimen.
Assist clients with navigating healthcare systems, insurance enrollment, and applications for public benefits or financial assistance.
Coordinate services by facilitating referrals to medical care, behavioral health providers, housing resources, and other community supports.
Advocate for clients with insurance carriers, social service providers, and other partners to reduce barriers to care.
Maintain accurate and timely documentation of client interactions, services, and referrals in accordance with agency policies.
Obtain and maintain required client consents, releases, and documentation.
Respond appropriately to client crises or urgent situations following agency protocols.
Prepare and submit required program documentation and monthly reports.
Maintain client confidentiality and comply with HIPAA and agency policies.
Maintain flexibility to work evenings, weekends, and varied schedules based on program needs.
Assist with coverage and support for other agency sites as needed.

What We’re Looking For

· Associate’s degree in human services or a related field required; bachelor’s degree preferred.

· 1–3 years of experience in human services or a related field strongly preferred

· 1-3 of case management experience preferred

· Strong organizational, problem-solving, and decision-making skills, with the ability to prioritize competing demands and meet deadlines.

· Excellent attention to detail and accuracy in documentation and client records.

· Knowledge of HIV/AIDS and related health and social issues preferred

· Working knowledge of local healthcare providers, social service agencies, and public entitlement programs preferred.

· Strong written and verbal communication skills with the ability to interact effectively with clients, colleagues, and community partners.

· Demonstrated ability to build effective, respectful, and supportive relationships.

· Ability to exercise sound judgment and maintain professionalism, integrity, and confidentiality.

· Proficiency in Microsoft Office (Word, Excel, Outlook) strongly preferred.

Walking/moving about work setting to accomplish tasks

Subject to climate-controlled office environments and requires flexibility to adapt to different environments as needed; employee is not substantially exposed to adverse environmental conditions
Recurring travel required; regular travel required during the first 3-6 months for training
Potential exposure to individuals experiencing medical or behavioral health crises, and to biohazard materials consistent with healthcare settings
Employees are expected to participate in and assist with at least two South Jersey AIDS Alliance fundraising events each year, including the NJ AIDS Walk.
