Full Job Description

A skilled nursing and rehabilitation center is seeking a dedicated and experienced Clinical Risk & Quality Coordinator (RN) to oversee the facility's clinical quality, risk management, regulatory compliance, and infection prevention programs.

This leadership role is responsible for promoting resident safety, improving clinical outcomes, ensuring regulatory compliance, and supporting a culture of quality throughout the organization. The ideal candidate is a proactive Registered Nurse with experience in long-term care, quality improvement, infection prevention, and performance management.

Clinical Risk & Quality Coordinator (RN) Key Responsibilities

Coordinate and support the facility's Quality Assurance and Performance Improvement (QAPI) program.
Monitor, track, and analyze clinical quality indicators, infection rates, and risk trends.
Lead the Infection Prevention and Control Program, including surveillance, investigation, tracking, and reporting of infections.
Implement and maintain infection prevention policies and procedures in accordance with CDC, CMS, OSHA, and state regulations.
Conduct outbreak investigations and coordinate response efforts with facility leadership and public health agencies when necessary.
Perform routine audits and observations to ensure compliance with clinical, quality, and infection control standards.
Provide education and training to staff regarding infection prevention, quality initiatives, risk reduction, hand hygiene, PPE usage, and regulatory requirements.
Investigate incidents, accidents, grievances, and other risk-related events, identifying opportunities for improvement.
Develop and implement corrective action plans to address identified concerns and improve resident outcomes.
Assist with survey preparedness and participate in state and federal regulatory surveys.
Prepare reports, maintain accurate records, and present findings to leadership and quality committees.
Collaborate with interdisciplinary teams to promote excellence in resident care and operational performance.

Clinical Risk & Quality Coordinator (RN) Qualifications

Current, unrestricted Registered Nurse (RN) license in the State of New Jersey.
Minimum of two years of nursing experience in a skilled nursing facility, long-term care, rehabilitation, or related healthcare setting.
Knowledge of quality assurance, infection prevention, risk management, and regulatory compliance.
Strong organizational, analytical, and problem-solving abilities.
Excellent communication and interpersonal skills.
Ability to manage multiple priorities while maintaining attention to detail.

Clinical Risk & Quality Coordinator (RN) Preferred Qualifications

Previous experience in Quality Assurance, Risk Management, Infection Prevention, Staff Development, or Clinical Leadership roles.
Experience with QAPI processes, quality measures, and regulatory survey preparation.
Certification in Infection Prevention and Control (CIC) preferred or willingness to obtain.
Experience analyzing healthcare data and developing performance improvement initiatives.
