Full Job Description

Job Summary

We are seeking a Part-Time Transition Specialist to join our team. The ideal candidate will support individuals with disabilities as they transition from school and other settings into community-based programs, employment opportunities, and support services. This position requires strong communication, organization, and interpersonal skills, as well as a commitment to promoting independence and successful community integration.

Key Responsibilities

Coordinate and support the transition process for high school students with disabilities through delivery of various transition services including job readiness classes, Smart Work Ethics, workplace tours, college tours, and job exploration.
Design new curriculum and training opportunities based on current trends in the labor market and teaching strategies.
Develop, implement, and monitor individualized transition plans based on each individual’s goals, strengths, and needs.
Collaborate with rehabilitation counselors, educators, families, and other service providers to ensure optimal scheduling and delivery of services.
Establish and maintain positive working relationships with individuals served, families, community partners, schools, and other stakeholders.
Assist individuals in identifying and accessing appropriate community resources, services, and opportunities.
Maintain accurate and timely documentation in accordance with program and organizational requirements.
Promote self-advocacy, independence, and successful participation in the community.

Qualifications

High school diploma or equivalent required.
Previous experience mentoring, teaching, coaching, or supporting youth is preferred.
Experience working with individuals with developmental disabilities is strongly preferred.
Strong computer and written communication skills, with proficiency in Microsoft Office and the ability to prepare clear, accurate written documentation.
Knowledge of basic financial management and budgeting principles.
Strong administrative, organizational, and documentation skills.
Ability to provide guidance, supervision, and leadership as needed.
Excellent time-management skills with the ability to prioritize multiple responsibilities.
Strong verbal communication, problem-solving, and interpersonal skills.
Ability to work effectively both independently and as part of a team.
