Social Worker Case Manager

Rx HealthCare Services is seeking a compassionate and motivated Social Worker Case Manager to join our Case Management department. This role will provide case management services to patients throughout Sacramento and Yolo counties.

Position Summary
The Social Worker Case Manager works closely with an RN as part of an interdisciplinary team to provide supportive, client-centered HIV/AIDS case management services through the AIDS Medi-Cal Waiver Program. This position serves a diverse client population in their home setting and focuses on helping clients remain stable, connected to care, and supported in the community.

Responsibilities

Partner with an RN to provide supportive, client-centered HIV/AIDS case management services in the home setting
Conduct biannual assessments and maintain monthly follow-up with clients between home visits
Assist clients with accessing and remaining connected to appropriate medical, mental health, AOD, and community-based services
Help stabilize clients’ health and social needs while working to avoid unnecessary higher levels of care
Maintain accurate and timely documentation of client interactions, assessments, and care coordination activities
Build trusting relationships with clients while providing compassionate and culturally sensitive support
Collaborate with community providers and internal team members to ensure continuity of care

Education/Training

Minimum of a Bachelor’s Degree in Social Work, Counseling, Psychology, or a related field
Equivalent degree qualifications may be considered

Experience

Minimum of 2 years of experience providing case management or related services
Experience working with HIV/AIDS clients or similar populations is preferred
Experience providing supportive services in home or community-based settings is preferred

Preferred Qualifications

Bilingual in English and Spanish preferred
Strong communication and interpersonal skills
Ability to work effectively with diverse populations
Compassionate, organized, and able to work both independently and as part of a team
