Full Job Description

WHO WE ARE

For almost 30 years, Signal has been managing and expanding behavioral health services in Colorado. Signal is a Behavioral Health Administrative Services Organization (BHASO) which helps ensure access to behavioral healthcare for uninsured, under-insured, and high-risk individuals by coordinating funding, supporting a provider network, and facilitating crisis response systems.

Signal's mission is to transform lives and communities by delivering high-quality and innovative professional services to behavioral health providers.

We are a non-profit organization dedicated to providing behavioral health services for youth, adults and families across Colorado. We manage mental health and substance use prevention, treatment and recovery services that are accessible, affordable, and efficacious for consumers of these services. Through effective management and coordination of services, Signal aims to increase efficiencies across behavioral health service systems, to strengthen linkages between human services systems and providers, and to more closely tailor services to the needs and strengths of consumers.

We also support Coloradoans through crisis services and crisis adjacent services for youth and families. Our youth and family service team helps to provide timely access to comprehensive clinical assessment and service linkage for youth with high acuity mental health needs.

WHAT WE ARE LOOKING FOR

The Care Access Coordinator will support Coloradans in need of behavioral health services and connection to community resources. This role will support Signal’s commitment to ensure availability and access to high quality behavioral health services across the state of Colorado. The Care Access Coordinator will serve as a vital member of the Clinical Services Department in acting as the “front door” for care coordination services, helping individuals navigate the behavioral healthcare system, and providing linkage to necessary care.

A day-in-the life as our Care Access Coordinator (Bilingual) may include:

Manage simultaneous incoming support requests from individuals, concerned others, and professionals with high quality customer service.
Conduct standardized screening of individuals’ needs related to mental health, substance use, and social determinants of health impacting their wellbeing.
Provide active and reflective listening as part of needs assessment and service and resource coordination. Utilize crisis management and de-escalation skills as needed.
Manage a dynamic caseload of clients, providing support for up to 30 days depending on the individual’s needs.
Engage in care navigation efforts on programs that support children, youth, and their families through Signal programs such as care access coordination and the I Matter program.
Support priority populations (e.g., youth and their families or caregivers, pregnant and parenting women, those involved with the criminal justice system, etc.) in linking to mental health health (MH) and substance use disorder (SUD) services.
Demonstrate extensive provider network knowledge in service to unique MH/SUD needs to include, but not limited to, culturally and linguistically appropriate services, gender-responsive services, and services along a continuum including prevention, treatment, and recovery.
Develop and maintain positive relationships with behavioral health providers, community organizations, and stakeholders to promote bidirectional referrals and increase awareness of the Care Access team.
Review and support the updating of the web-based directory of services.
Problem-solve with the individual/care team around barriers impacting access to care and improving wellbeing.
Facilitate enhanced (warm) referrals to connect the individual to their insurer, treatment provider, and community resource agency, or systems partner.
Intra- and cross-department coordination related to ongoing care delivery needs through safety net systems of care.
Provide timely enhanced referral coordination for those requesting assistance.
Timely documentation of all care navigation efforts in Signal's systems.

Qualified candidates will possess:

Experience in behavioral healthcare service delivery and coordination of care across systems.
Strong organizational skills and communication skills in all areas including written, verbal, one-on-one, and family settings.
Excellent customer relations skills.
Ability to handle confidential and sensitive information with respect and care for the person.
Knowledge of mental health and substance use delivery systems including State and/or local government programs, behavioral healthcare entities, and related organizations.
Good judgment, tact, initiative, and diplomacy to make effective independent decisions.
Organized, attention to details, and ability to manage a variety of tasks at the same time.
Ability to work in a team setting and receive and provide constructive feedback.
Proficiency in Microsoft Excel, Outlook, Word, and PowerPoint and ability to learn other electronic systems.
Ability to pass background check

Bilingual Care Access Coordinator

Will possess all the above required skills and credentials as well as be proficient in Spanish. Testing for proficiency may be required. Additional compensation for this proficiency is available.

Education

High School equivalency required.
Bachelor’s degree in social or human services field, preferred; other degrees also acceptable contingent upon experience.
