Pre-Procedural Services Pharmacy Admission Specialist (Medication Reconciliation Technician)

Location: UCHealth Admin 2450 Peoria, Aurora, CO

Department: Pre-procedure Services

Work Schedule: Full Time, 80.00 hours per pay period (2 weeks), Monday - Friday, 8:00am - 4:30pm (1 in 4 weekends) and rotation through holidays

Shift: Days

Pay: $22.14 - $28.78 / hour. Pay is dependent on applicant's relevant experience

This position is an onsite role and does not offer a hybrid or remote option

Summary:

Pharmacy Admissions Specialists (PAS) are responsible for helping the healthcare team by obtaining and documenting patient's home medications in the electronic health record. This is also known as a "Med Rec Tech" at other institutions. The primary focus of this position will be calling patients ahead of their scheduled visit with the Pre-Procedural Services Clinic to accurately update their medication list.

This position requires staff to effectively use computers and interact with patients over the phone, working from a call-center for the entirety of a shift. We are seeking team members who are self-motivated to complete work and maintain a fast pace without regular oversight. Ideal candidates for this position include technicians who are looking to stay in a position for several years and already have experience with medication history documentation or time.

This role does not include staffing within our outpatient pharmacies (retail) or inpatient pharmacy (IV room, medication delivery, etc.).

Responsibilities:

Interview patients and family members to obtain a valid list of patient's home medications upon admission or prior to their PPS clinic visit.
Communicate with retail pharmacies, primary care physicians, and other resources to gather information and recognize and apply knowledge of appropriate/specific drugs within scope.
Accurately record information in the patient's electronic health record in a timely manner and maintain accurate data involving patient information.

Requirements:
High School diploma or GED.

Pharmacy Technician certification from Colorado Board of Pharmacy (PHAT).
Pharmacy Technician Certification (CPhT) by Pharmacy Technician Certification Board (PTCB) or equivalent.
