Job Summary

Home Health Case Manager coordinates care for children and adults with disabilities to help them thrive and live their best life! The Case Manager utilizes the nursing process to advocate for medically fragile indivduals to bridge the gap in care and promote improved outcomes. Step beyond the bedside and experience the rewarding nursing specialty of Home Health Case Management as a hybrid remote long-term home health case manager serving the Denver Metro Area. LiliSare supplies the equipment you need to be successful including hourly pay, Remote access, computer, technology package and nursing equipment with PPE. Schedule patients to be seen in their home with paid travel time and mileage reimbursement. Travel is required 40% of the time and the majority of the Case Manager responsibilities are completed from home via electronic medical record and other remote communication allows for a flexible hybrid work-from-home schedule! Leadership and professional growth opportunity!

Responsibilities:

Assessment of needs, development and implementation of plan of care and care coordination
Complete documentation as required using the EMR Reviews clinical documentation to ensure its completeness
Communication and collaboration with multiple disciplines and supervisor

Requirements:

Active Colorado RN license
Basic Life Support (AHA accredited)
1 year bedside nursing or 1 year Case Management experience
COVID-19 vaccination or waiver required
Reliable transportation and valid vehicle insurance is required (for travel to patients)
Internet access and phone service (phone reimbursement available)

Skills:

Analytical thinking, Clinical decision making and Problem solving.
Ability to prioritize and organize workload independently
Strong written and Oral communication abilities
Electronic charting and basic computer skills - excel, Word and pdf
