RN - Hospice Admissions

We are seeking a dedicated and compassionate Registered Nurse specializing in Hospice Admissions to join our healthcare team. The ideal candidate will possess a strong background in patient assessment, clinical case management, and hospice care coordination. This role offers an opportunity to provide critical support to patients and their families during challenging times, ensuring comfort, dignity, and personalized care. The Registered Nurse will serve as a vital link between patients, families, and interdisciplinary healthcare teams, facilitating smooth admission processes and comprehensive care planning.

Duties

Conduct thorough patient assessments to determine eligibility for hospice services and develop individualized care plans.
Manage the admission process by coordinating with physicians, social workers, and other healthcare professionals to ensure seamless onboarding of patients.
Educate patients and families about hospice services, expectations, and available resources with clarity and compassion.
Document all clinical findings accurately within Electronic Health Records (EHR) systems while adhering to HIPAA regulations.
Collaborate with interdisciplinary teams to facilitate appropriate symptom management, discharge planning, and ongoing patient support.
Monitor vital signs and assess physiological status regularly to adjust care plans as needed for optimal comfort.
Participate in case management activities including diagnostic evaluation, utilization management, and post-acute care coordination.
Maintain compliance with health regulation policies and ensure all documentation meets regulatory standards for hospice care.

Skills

Extensive experience in critical care settings such as ICU, Emergency Room, or Level I/II Trauma Centers is highly desirable.
Proven expertise in hospital medicine, medical-surgical nursing, or home health environments with a focus on acute and post-acute care.
Strong knowledge of physiology, medical terminology, infectious disease care, and behavioral health principles.
Demonstrated ability to manage complex cases involving geriatrics, pediatrics (PICU experience), or chronic illness populations.
Proficiency in Electronic Health Records (EHR) management systems along with familiarity with HIPAA compliance standards.
Excellent patient service skills combined with effective communication for client interaction and team collaboration.
Ability to perform comprehensive patient assessments, manage discharge planning processes, and coordinate outpatient or clinic-based services.
Knowledge of health regulation policies related to hospice care, managed care practices, utilization management, and diagnostic evaluation procedures.
