Job Description:

Conduct thorough initial evaluations with patients.
Ensure every patient receives comprehensive and individualized behavioral health sessions.
Provide continuous communication with the team and referring physicians.
Implement health treatment programs based on each patient's particular needs focusing on diet change.
Accurate and timely documentation of patient encounters, counseling sessions, progress and discharge notes.
Assist in marketing to new and existing referral sources
