General Description

The Family Support Specialist/Community Engagement Coordinator is responsible for the overall case management of all services provided to families enrolled in the Early Head Start Partnership Program and Preschool for All/PFA-Expansion. A working knowledge and skill in relationship-based engagement with families is required and implementation of the Head Start Performance Standards is a plus.

Duties and Responsibilities

Conduct and assist with enrollment and parent interviews via in-person, telephone, or online methods.
Responsible for collaborating with partner locations to identify and recruit the most at-risk children in the community, including the recruitment of children with disabilities.
Establish and maintain accurate documentation of eligibility for all children.
Document and provide follow-up for children’s absences in MyHeadStart and IWAS systems.
Collaborate with families to complete an accurate Family Assessment annually and provide intervention, referrals, and assistance for interests, needs and circumstances identified as well as assistance with referrals within their communities, i.e., emergency services, health/nutrition/dental referrals, education/ mental health/disability referrals, and child abuse and neglect referrals, etc.
Assist families in identifying and defining their goals, and ensure that these goals are reviewed and documented in MyHeadStart.
Ensure that all families and children are linked to a medical and dental home for ongoing, continuous service.
Obtain, track, and document required documents related to health and dental for EHS and PFA/PFA-E compliance guideline areas.
Maintain accurate and current record-keeping of the family’s file as outlined by the file folder checklist.
Plan and facilitate two parent conferences by sharing developmental information from the family file to support relationships with families.
Plan and facilitate monthly parent committee/policy council meetings and parent workshops that provide training to parents on community resources and services.
Monitor reports in MyHeadStart and the IWAS systems, and track forms according to established procedures.
Plan and collaborate on monthly family engagement activities with team members to enhance family engagement.
Create and distribute monthly newsletters for parents and families.
Work with partner childcare sites to ensure that parents have access to parent/child libraries.
Collaborate with EHS staff to keep the Community Resource Guide updated and create a PFA/PFA-E Community Resource Guide with directors.
Recruit and assist with the coordination of local Health Services Advisory Committee members and monthly and/or bi/monthly meetings for all childcare partners.
Attend and participate in staffing of children in collaboration with the Local Interagency Council (LIC), mental health provider, and/or medical agency.
Participate in collecting data for the community assessment.
Participate in the monitoring system designed for the CCR&R Early/Head Start Partnership Program and PFA/PFA-E, ensuring follow-up on all issues identified during technical assistance and monitoring visits.
Respond appropriately to cultural diversity and be sensitive to the needs of the families and community served.
Attend regularly scheduled meetings, Grantee service meetings/trainings, Pre-Service and In-Service Training.
Must demonstrate belief in the CCR&R mission and vision in all verbal and written communication.
Meet all timelines and reporting requirements as outlined by CCR&R, Head Start Performance Standards, ISBE, and DCFS.
Participate in scheduled CCR&R, Early Head Start, and ISBE training, and other agency or job-related conferences.
Ensures that the use of agency technology, equipment, and time is utilized for agency business.
Performs other duties as may be assigned.
