About OffPlan

OffPlan is a direct care platform that helps employers, employees, individual consumers, and physicians move everyday healthcare, primary care, chronic disease management, preventive visits, labs, and care coordination, out of the insurance system entirely. Built in partnership with independent, physician-owned primary care practices, OffPlan gives members same-day or next-day access to a dedicated physician through a straightforward membership: no claims, no prior authorizations, no deductibles. Insurance and stop-loss coverage remain in place for the roughly 10-15% of care that is genuinely catastrophic, so insurance functions in the background as intended.

OffPlan primarily serves small and mid-size employers, including self-funded, level-funded, and fully insured groups, replacing high-cost, underused insurance plans with a fixed-cost, physician-led model that delivers substantial savings and better care. Alongside the employer business, OffPlan operates a direct-to-consumer channel that lets individuals join a physician practice directly, and this D2C membership base is a core, growing part of the business. Physicians in the OffPlan network own their practices, carry smaller patient panels, spend more time with each patient, and offer responsive, personalized communication.

OffPlan is an early-stage, founder-led company moving fast toward its Florida and Virginia launch, working with employers, benefits advisors and brokers, individual consumers, and physicians who want to practice medicine without an insurance company standing between them and their patients.

The Role

OffPlan is hiring its first Customer Success / Onboarding Manager to own the member and employer-group experience from the moment a company signs on, or an individual joins through the D2C channel, through every day of active membership. This is the person who makes sure everything the sales and benefits conversations promise, no claims, no prior authorizations, same-day access, actually happens smoothly for real members. You will manage new employer group implementations, onboard individual D2C members, guide members through enrollment, and serve as the front line for care coordination issues that need a human touch.

Day to day, this role monitors engagement and satisfaction across all member populations, analyzes usage and outcomes data, prepares reports and recommendations, and collaborates with physicians, marketing, and the founding team to refine processes and service offerings. The role also educates clients and members about OffPlan's model, facilitates feedback loops, and drives retention by demonstrating value and maintaining strong, trust-based relationships.

Key Responsibilities

Own the end-to-end onboarding experience for new employer groups, from signed agreement through first-member enrollment, coordinating with physicians, brokers, and the employer's HR team
Own onboarding and early engagement for individual D2C members, building a self-serve enrollment path distinct from the employer-group process
Serve as the primary point of contact for member questions about eligibility, the benefit card, curated specialty access, and how to use OffPlan day-to-day
Build and manage onboarding materials, welcome sequences, and FAQs that make the model easy to understand for first-time members, across both employer-group and D2C populations
Monitor member satisfaction and retention metrics across all populations, using data analysis to identify trends and flag patterns that need attention from physicians, employers, or the founding team
Coordinate with physician practices to ensure smooth patient panel onboarding and resolve care coordination issues that fall outside routine visits
Act as the internal voice of the member and employer, feeding recurring issues back to product, marketing, and operations
Manage the customer care support process as OffPlan launches its first employer groups and D2C members in Florida and Virginia
Partner with the Marketing Manager to ensure onboarding communications match brand voice and messaging

What You Bring

4+ years in customer success, onboarding, account management, or customer care, ideally in healthcare, benefits, insurance, or another regulated services category
Experience managing implementation or onboarding for B2B2C products, where the buyer (employer) and end user (employee/member) are different people with different needs; direct-to-consumer onboarding experience is a plus
Customer satisfaction and customer service skills to ensure clients and members have a positive, consistent experience
Customer retention and relationship-building skills to foster long-term partnerships with employers, individual members, and physician practices
Analytical skills to interpret engagement data, identify trends, and recommend improvements for program performance
Strong communication and presentation skills to clearly explain OffPlan's model and convey complex information to diverse audiences
Comfort being the calm, clear voice on the other end of a confusing healthcare or benefits question
Strong organizational skills; the ability to run multiple employer group launches and member accounts at once without dropping details
A genuine service mindset; real satisfaction in solving someone's problem, not just closing a ticket
Comfort with technology platforms and basic data tools; familiarity with CRM systems is beneficial
Direct primary care, self-funded/level-funded benefits, or health insurance experience is a strong plus
Bachelor's degree in Business, Healthcare Administration, Communications, or a related field, or equivalent practical experience

Compensation

$70,000 to $85,000 base salary, plus options eligibility. This range reflects current Atlanta metro benchmarks for customer success/onboarding roles.
