Responsibilities (RN & LPN)
- Provide home-based skilled nursing care for patients recovering from illness, injury, or surgery, or receiving hospice care.
- Conduct patient assessments and document findings in accordance with agency policies and care plans.
- Collaborate with physicians, therapists, and the interdisciplinary care team to ensure comprehensive, high-quality care.
- Educate patients and caregivers on disease processes, medications, diet, and symptom management.
- Communicate changes in patient condition promptly to supervising RN (for LPNs) or physician (for RNs).
- Follow infection control and safety protocols at all times.
Additional Responsibilities (RN Only)
- Complete admission, recertification, and discharge assessments as assigned.
- Supervise and provide direction to LPNs and Home Health Aides as required by agency policy and State/Federal regulations.
- Participate in interdisciplinary team meetings and plan of care reviews.
Qualifications
Registered Nurse (RN):
- Graduate of an accredited nursing program and active RN license in Michigan
- Home Health or Hospice experience preferred
- Current CPR certification
- Valid driver’s license and reliable transportation
Licensed Practical Nurse (LPN):
- Graduate of an accredited LPN program and active LPN license in Michigan
- Prior experience in home health, hospice, or geriatric care preferred
- Current CPR certification
- Valid driver’s license and reliable transportation
Travel to patient homes across the Thumb and surrounding counties is required.
