Job Overview

We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. In this vital role, you will provide psychosocial support, counseling, and resource coordination for patients in home health settings. Your expertise will help patients and their families navigate complex medical situations, emotional challenges, and social needs, ensuring comprehensive care that promotes well-being and quality of life. This paid position offers an exciting opportunity to make a meaningful impact through clinical social work practice.

Duties

Conduct thorough patient assessments to identify psychosocial needs, including mental health status, family dynamics, and community resources.
Provide individual and group counseling sessions utilizing evidence-based techniques such as cognitive behavioral therapy, motivational interviewing, and behavioral therapy.
Coordinate with interdisciplinary teams to develop personalized discharge plans and ensure seamless transition of care.
Assist patients and families in understanding medical diagnoses, treatment options, end-of-life care preferences, and available community resources.
Manage case documentation accurately within electronic medical records (EMR) systems while adhering to HIPAA regulations and clinical ethical guidelines.
Facilitate crisis intervention and provide immediate support during emergencies or acute mental health episodes.
Collaborate with healthcare providers on clinical research initiatives related to social determinants of health or specific patient populations such as geriatrics or individuals with disabilities.

Experience

Demonstrated expertise in medical social work practices including discharge planning, intake assessments, and case management.
Familiarity with medical terminology, ICD coding, Medicare policies, and medical documentation standards.
Knowledge of EMR systems for efficient documentation and data collection; experience working with individuals from diverse cultural backgrounds preferred.
Certification in basic life support (BLS) and CPR required; additional certifications in addiction counseling or behavioral health are advantageous.
