Full Job Description

As a Navigator / Advocate at Claramente, you are the personal guide our members rely on. You will work directly with seniors to solve real problems — whether that means tracking down a specialist, finding a prescription assistance program, arranging a ride to a doctor's appointment, or simply helping someone understand what their Medicare plan actually covers.

This is a hands-on, problem-solving role. You won't just hand out referrals — you'll follow through, make calls, remove barriers, and stay with clients until the issue is resolved.

What You'll Do

Care Navigation & Problem-Solving

Conduct intake calls to understand each member's needs.
Serve as a dedicated personal navigator for an assigned caseload of senior members
Proactively identify barriers to care and find creative, practical solutions
Connect members to the right doctors, specialists, and facilities — and follow through to make sure they get there
Coordinate referrals, appointments, and specialist visits end-to-end
Check in with members regularly to monitor progress and address new needs

Resource Connection

Identify and connect members to prescription assistance programs, copay support, and pharmacy partnerships to reduce drug costs
Coordinate covered transportation to medical appointments so distance is never a barrier
Connect members to mental health counseling, wellness resources, and emotional support services
Maintain an up-to-date knowledge of local and national resources available to Medicare beneficiaries

Medicare Benefits Guidance

Help members understand and maximize their Medicare and Medicare Advantage plan benefits
Assist members in avoiding coverage gaps, missed benefits, and unnecessary out-of-pocket costs
Explain insurance processes, referrals, and prior authorizations in plain language (English and Spanish)

Advocacy & Follow-Through

Advocate on behalf of members within the healthcare system when they face obstacles
Make calls, send follow-ups, and escalate issues as needed — don't stop until the problem is solved
Conduct weekly check-ins with members, especially following surgeries, hospitalizations, or major health events
Document all interactions and outcomes accurately in our case management system

Who You Are

A natural problem-solver who doesn't give up when the first door closes
A compassionate communicator who makes seniors feel heard, respected, and cared for
Bilingual in English and Spanish (required)
Knowledgeable about or eager to learn Medicare, Medicaid, and community health resources
Organized, reliable, and able to manage a caseload independently
Comfortable on the phone — most of our work happens via calls and follow-ups
1–2 years of experience in healthcare navigation, social services, community health, or a related field preferred; equivalent lived experience considered
