We are seeking a compassionate and dedicated Patient and Family Care Liaison to join our healthcare team. In this role, you will serve as a vital link between patients, their families, and healthcare providers, ensuring clear communication, emotional support, and coordination of care. You will advocate for patients’ needs, facilitate understanding of treatment plans, and help navigate healthcare services to enhance the overall patient experience.

**Key Responsibilities:**

Act as the primary point of contact for patients and their families, addressing concerns and providing timely information.
Collaborate with medical staff to coordinate patient care and discharge planning.
Educate patients and families about treatment options, hospital policies, and available resources.
Support patients and families emotionally during challenging times.
Assist in resolving conflicts and improving patient satisfaction.
Maintain accurate records of patient interactions and care coordination activities.

Proven experience in patient advocacy, social work, nursing, or a related healthcare field.
Excellent communication and interpersonal skills.
Strong problem-solving abilities and empathy.
Ability to work collaboratively within a multidisciplinary team.
