Full Job Description

Job Overview
The Case Manager works directly with clients to complete assessments, implement service plans, provide referrals to appropriate services. Works as a member of the Women's Housing Coalition team to provide the highest quality of services to residents. This position reports to the Program Director.

Duties

Client Services: Work with directly with clients to complete assessments, implement service plans, provide referrals to appropriate services. Assist clients in reaching their goals and the goals set by the WHC. Monitor clients for compliance and growth, and evaluate if the WHC remains an appropriate placement for the client. Attend and participate in clinical meetings and case management training sessions as required.

Maintain separate files on all dependent children in the program to include but not limited to, school attendance records, IEP plans/reports, participation in mental health or physical health services. Complete all necessary assessments. Assist with the coordination of summer camp activities for all children. Work with the Program Director to monitor the needs of the children and identify possible programming opportunities and partnerships that would facilitate the children’s well-being.

Compliance:

Enter all required data into HMIS within 12 hours of contact. Enter all required data into Clienttrack by due dates. Understand that data collection should be subjective from case manager’s professional interpretation and is not just client self-reported information. Client files and data collection software information will be maintained and audit ready at all times; and follow the WHC guidelines.

Administrative:

Ability to properly use all office equipment: cell phones, desk phone, copiers, scanners, fax, and computer. Ability to use Microsoft Office: Outlook including calendar and meeting function, Word, Excel. Ability to properly use software including Clienttrack, ETO, ADP, Boston Post. Ability to access, save, and properly store documents in the correct network files. Attend and participate in staff meetings and training sessions as required.

Property:

Conduct unit inspections for cleanliness and client safety. For Linden House, generate work orders for any maintenance and repairs for building issues noticed. Be present for annual fire drills. When necessary, be available to allow vendors and inspectors access to the building. For Scattered Sites, notify Property Manager of any maintenance and repair issues that should be addressed. Assist Scattered Sites Assist resident in the identifying of available units.

Requirements

Bachelor’s Degree in human/social service field. At least three years prior case management experience.

· Understanding of ethical behavior and commitment to ensure own behavior and the behavior of others is consistent with these standards.

· Ability to establish and maintain positive working relationships with others both internally and externally to achieve the goals of the organization.

· Ability to speak, listen and write in a clear, thorough and timely manner using appropriate and effective communication tools and techniques.

· Ability to multi-task and prioritize assignments in a changing environment.

· Ability to set challenging goals for self and others; willingness to take personal accountability for delivering high-quality results, take on additional responsibilities when needed, and persist in the face of obstacles. Excellent written and oral communication skills. Must be a self-starter and able to work with minimal supervision.

Solution orientation: Ability and willingness to assess problem situations in order to identify causes, gather and process relevant information, and generate possible solutions to resolve the problem.

Pay: $24.00 - $25.00 per hour
