Targeted Case ManagerHelp People Find Housing. Build Stability. Create Possibilities.

Are you passionate about helping people overcome barriers and build a more stable future? Do you enjoy connecting people with resources, building relationships, and celebrating progress—both big and small?

We are looking for a compassionate, motivated, and resourceful Targeted Case Manager to join our team and support adults experiencing homelessness.

In this rewarding role, you'll work alongside clients to help them secure safe and stable housing, connect with mental health and healthcare services, access community resources, and achieve their personal goals.

You'll be more than a case manager—you'll be an advocate, connector, problem-solver, and source of encouragement for the people you serve.

What You'll Do

As a Targeted Case Manager, you will:

Build trusting, professional relationships with clients and their support systems.
Help clients experiencing homelessness identify and secure safe, stable housing.
Connect clients with mental health, healthcare, and community-based services.
Collaborate with healthcare providers, service agencies, landlords, community organizations, and other partners.
Regularly communicate with providers involved in client care to ensure services are coordinated and client needs are being addressed.
Complete comprehensive assessments and identify client needs.
Develop individualized care plans and crisis plans.
Complete ANSA assessments every 90 days, as required.
Make referrals and monitor the delivery of services.
Coordinate services and help clients navigate available resources.
Monitor client safety, well-being, stability, and progress toward identified goals.
Empower clients to build upon their strengths, resources, and natural supports.
Provide services using trauma-informed, person-centered, and strengths-based practices.
Respect each client's privacy, individuality, choices, culture, and personal goals.
Maintain appropriate professional boundaries and ethical standards.
Maintain accurate and timely client records in accordance with agency policies, applicable laws, licensing requirements, MaineCare regulations, and client rights.
Independently manage an assigned caseload, including scheduling appointments, meetings, community visits, and documentation time.
Participate actively in supervision, training, and professional development.

What We're Looking ForRequired Qualifications

Bachelor's degree in Psychology, Mental Health & Human Services, Social Work, Nursing, Social & Behavioral Sciences, Sociology, Education, or a related field.
Strong written and verbal communication skills.
Reliable transportation and valid vehicle insurance.
Ability to work with clients in their homes and throughout the community.
Ability to work independently while also being an effective member of a collaborative team.
Commitment to completing required annual training, including 20 hours of case management training.

Preferred Qualifications

At least two years of case management experience.
Knowledge of Section 13 of the MaineCare Benefits Manual.
Experience working with individuals experiencing homelessness, mental health challenges, or barriers to accessing healthcare and community resources.
Working knowledge of Microsoft Office, Microsoft Outlook, Zoom, and other video conferencing platforms.
