Cornerstone Youth and Family Services is seeking a Wraparound Facilitator to help youth with complex needs remain safely connected to their families, schools, and communities.
This is more than case management. You will bring families, providers, schools, community resources, and natural supports together to create a coordinated plan centered on the family's strengths, needs, goals, and vision for the future.

Build trusting relationships with youth and families.

Facilitate Child and Family Team meetings.

Develop individualized Plans of Care with families and their teams.

Connect families with behavioral health, educational, social-service, and community resources.

Advocate for youth and families and ensure their voices remain central.

Coordinate with providers, schools, Family Support Partners, Youth Support Partners, and community resources.

Support crisis planning and strategies that promote safety and stability.

Manage a caseload of approximately 10 youth/families.

Complete timely and accurate documentation while maintaining confidentiality and required compliance standards.
Required:
-
Bachelor's degree in Social Work, Human Services, Behavioral Sciences, or a related field; or a bachelor's degree in another field with at least two years of relevant experience; or an LDH-approved equivalent.

Strong communication, relationship-building, organization, and documentation skills.

Ability to facilitate meetings and collaborate with families and multiple professionals.

Valid driver's license, reliable transportation, and ability to travel throughout the service region.

Ability to work a flexible schedule, including occasional evenings and weekends.
Preferred:
-
Master's degree in Social Work, Counseling, Psychology, Behavioral Health, Human Services, or a related field.

Experience working with youth, families, behavioral health, mental health, case management, or community-based services.

Experience coordinating services across multiple systems or with crisis prevention/intervention.

Employment is contingent upon successful completion of applicable background screenings, drug screening, TB testing, eligibility requirements, required First Aid/CPR/seizure training, and Wraparound training and credentialing. Candidates must meet applicable Medicaid/Medicare participation requirements and have no applicable sanctions, exclusions, or adverse-action findings.
