Overview
Join our dynamic healthcare team as a Community Care Navigator, where your passion for patient advocacy and community support will make a meaningful difference. In this vital role, you will serve as a bridge between patients and the healthcare system, helping individuals access the services they need to improve their health outcomes. Your energetic approach and compassionate communication skills will empower underserved populations, including those with mental health conditions and co-occurring disorders, to navigate complex healthcare landscapes confidently. This paid position offers an exciting opportunity to contribute to community well-being while developing your expertise in medical coding, case management, and social services.

Responsibilities

Assist patients in understanding and accessing healthcare services, including behavioral health, pediatrics, and acute care support
Conduct intake assessments and coordinate discharge planning to ensure seamless transitions of care
Maintain accurate medical records and manage electronic health records (EHR) with precision and confidentiality
Educate clients on Medicaid regulations, health insurance options, and patient rights while ensuring compliance with HIPAA guidelines
Collaborate with healthcare providers, social workers, and community organizations to develop personalized care plans
Support data entry tasks related to client information, service utilization, and case management documentation
Communicate effectively with clients from diverse backgrounds, including bilingual populations, to foster trust and engagement
Work within healthcare support frameworks to assist individuals with mental health conditions or co-occurring disorders in accessing appropriate resources

Skills

Proven experience connecting patients with community resources and plan benefits
Strong knowledge of medical coding systems such as CPT, ICD-9, ICD-10, and proficiency in electronic health record (EHR) management
Background in community health or behavioral health services with experience supporting individuals from underserved communities
Excellent client communication skills with the ability to handle sensitive information discreetly while maintaining patient confidentiality under HIPAA regulations
Ability to perform detailed data entry accurately and efficiently in fast-paced healthcare settings
Familiarity with Medicaid regulations, health insurance processes, discharge planning procedures, and medical records management
Experience working with individuals who have mental health conditions or co-occurring disorders is preferred
