Intake Assessor (LSW/LPC)

Location: Dayton, OH

Schedule: Full Time

Pay Range: $57 000 - $67 000 per year

Licensure Required: LISW, LSW, LPC, or LPCC

We are seeking a dedicated Intake Assessor to serve as the first clinical point of contact for patients seeking treatment. This role is ideal for a licensed behavioral health professional who excels in assessment, communication, and collaboration while helping individuals access the care they need.

Position Summary

The Intake Assessor is responsible for conducting comprehensive behavioral health assessments, determining appropriate levels of care, coordinating admissions, and ensuring a seamless transition into treatment. This position works closely with patients, families, referral sources, and the clinical team to provide an exceptional admissions experience.

Key Responsibilities

Conduct comprehensive biopsychosocial and behavioral health assessments.
Evaluate patient needs and determine appropriate treatment recommendations and level of care.
Coordinate admissions and facilitate timely placement into appropriate programs.
Schedule and conduct intake appointments and follow-up communications.
Maintain accurate and timely clinical documentation in accordance with organizational, state, and regulatory standards.
Respond to inquiries from prospective patients, families, healthcare providers, and community referral sources.
Collaborate with multidisciplinary clinical teams to support continuity of care.
Build and maintain positive relationships with referral partners and community stakeholders.
Ensure compliance with confidentiality standards, HIPAA regulations, and clinical best practices.
Participate in team meetings and contribute to continuous quality improvement initiatives.

Qualifications

Required

Active Ohio licensure as one of the following:
- LISW
- LSW
- LPC
- LPCC
Strong clinical assessment and interviewing skills.
Excellent communication, organization, and documentation abilities.
Ability to work independently while collaborating effectively within a multidisciplinary team.

Preferred

Experience in behavioral health, mental health, substance use treatment, or crisis assessment.
Familiarity with electronic health records (EHR) systems.
Knowledge of ASAM criteria and behavioral health admissions processes.
