Job Title: Lead Housing Case Manager

Location: Bellingham, WA Light travel will be required.

Term and Salary: Pay is dependent upon experience ranging from $27-32/per hour. This is a full-time, 40 hours per week, non-exempt position.

About Road2Home: Road2Home is a 501(c)(3) nonprofit organization based in Bellingham, WA providing community-based support for people experiencing homelessness. Our goal is to help our clients access services and develop strategies that enable them to meet their basic needs while on a path to stable housing. Our Outreach Department provides direct services to people experiencing homelessness, and our Shelter Program provides temporary housing and case management services.

About Gardenview Village: Gardenview Village is a 35-unit tiny home community serving medically fragile and aging adults experiencing homelessness. The program provides low-barrier shelter, individualized case management, housing navigation, and connection to medical and behavioral health resources. The environment is collaborative, field-based, and centered around client choice and wellbeing.

Job Summary: The Lead Case Manager provides intensive case management to a caseload of approximately 10-15 residents while also serving as the primary supervisor for the case management team. This position supports residents in transitioning to permanent housing, oversees program reporting requirements, and ensures consistency of care aligned with Road2Home values and contractual expectations. The Lead Case Manager also holds internal decision-making authority related to case planning, behavioral interventions, and resident support strategies

Essential Duties and Responsibilities:

Provide direct services to clients based on individuals' needs, including but not limited to; assessments, referrals, and resource viability and acquisition.
Provide services, using a strengths-based and trauma-informed approach, to a caseload of about 10-15 clients with a case mix of individuals who are primarily over the age of 55, who are medically fragile and are unsheltered. This position will help clients meet their short and long-term goals and work towards permanent and long-term housing.
Advocate for and serve as a liaison for clients and potential housing opportunities, assist in completing paperwork, applications, and develop an individual and permanent housing plan. This may include assisting with deposit and rental assistance paperwork and coordinating with Whatcom Homeless Service center for rental subsidies, in addition to coordinating with other agencies.
Provide conflict management and behavior plans for clients when their housing may be at risk.
Assist in overcoming housing barriers such as lack of documentation, rental history, or evictions.
Assist in the coordination of care for clients through collaborative efforts with external service providers and case management partners to provide comprehensive case management.
Process and maintain required information and paperwork, including confidential case notes, client records, and personal data. Must comply with HIPAA to maintain the confidentiality of all data and client interactions in accordance with federal, state, and agency guidelines.
Provide reporting information for data management systems and comply with all contract requirements.
Assist in coordinating safety and crisis responses for clients when needed.
Provide basic skills training and empower clients in the advocacy of their needs.
Facilitate groups, educational events on occasion, and participate in the facilitation of special events and holiday celebrations.
Accurately document case management notes, progress on goals set, and services provided in a timely manner.
Attend training to satisfy licensing criteria, and engage in professional development opportunities.
Other duties as assigned.
Physical demands for this position may include but are not limited to driving, sitting, standing for long periods, and occasionally lifting up to 50lbs.

Additional Lead Duties:

Supervise two Case Managers, including scheduling, timesheet approval, annual reviews, corrective action, and professional development
Provide policy and procedural oversight and guidance to direct reports.
Engage in the development, implementation, and monitoring of training and onboarding for new hires/direct reports.
Attend meetings related to case management projects.
Take lead on new case management efforts, as needed.
Other duties as assigned.

Qualifications and Requirements:

BA in Human Services or related field preferred. Other degrees combined with relevant or demonstrated experience will be considered.
Minimum 2 years of case management experience.
Minimum 3 years of related experience (housing, behavioral health, social services, or homelessness response).
Must have knowledge of or experience with trauma-informed care and harm reduction approaches.
Must possess basic knowledge of the housing system and support services for vulnerable and unsheltered individuals.
Ability to read and interpret documents and effectively communicate verbally and in written form with clarity. Must be able to communicate and maintain relationships with clients, colleagues, supervisors, and community partners. Excellent public speaking skills required.
Have proficiency in Microsoft Office and Google Workspace, and a general comfort and proficiency with technology.
Must be organized, flexible, and responsive, and able to complete work in a timely manner, sometimes under complex or changing timelines.
Must be able to work well in a team and embrace inclusion and diversity amongst colleagues and clients.
The ideal candidate will be able to work independently, handle stressful situations effectively, understand and be sensitive to various life circumstances of individuals they encounter, including trauma, racism, homelessness, violence and abuse, poverty, and mental and physical health status.
Must have a valid state-issued driver's license or obtain by start date.
Must provide a recent driving abstract for the last three years by start date.
Must be able to pass a Federal background check.
